DESCRIPTION/JUSTIFICATION OF AMENDMENT, MODIFICATION, OR CHANGE ORDER: <br /> To provide U.S. Department of Homeland Security (DHS) Emergency Management Performance Grant <br /> (EMPG) funds to local jurisdictions and tribes that have emergency management programs to support <br /> their emergency management operating budget in an effort to enhance their emergency management <br /> program. <br /> The Contractor, City of Everett Emergency Management, requests an extension to fully expend <br /> remaining funds and ensure all equipment purchased under this contract has been received, installed, <br /> and tested. Heavy snow in December prevented employees from getting to work, which delayed <br /> progress on this contract. Severe flooding in January caused all emergency management personnel <br /> out of routine work to focus on coordination of assistance to Snohomish County. The delays in <br /> processing EMPG grant order requests, vetting of equipment and approval processes were delayed for <br /> two months. <br /> 11. AMENDMENT TERMS AND CONDITIONS: <br /> 1. The overall contract am ount of$66,144 remains unchanged. <br /> 2. Change the contract end date from March 31, 2009 to August 31, 2009. <br /> No other changes are required. All other terms and conditions of the original contract and any <br /> previous amendments thereto remain in full force and effect. <br /> IN WITNESS WHEREOF, the Department and Contractor acknowledge and accept the terms of this contract amendment <br /> and attachments hereto and in witness whereof have executed this amendment as of the date and year written below. <br /> The rights and obligations of both parties to this contract are governed by this Contract Amendment Face Sheet and other <br /> documents incorporated herein by reference or attached and identified in the original contract document. A copy of this <br /> contract amendment shall be attached to and incorporated into the original agreement between the Department and the <br /> Contractor.
