EXHIBITA <br /> ki <br /> SPECIFIC TERMS AND CONDITIONS <br /> SENIOR CENTERS <br /> DOCUMENTS INCORPORATED BY REFERENCE <br /> In performing the services under this Contract, City of Everett, a Washington <br /> municipal corporation (the "Contractor"), shall comply with the following documents <br /> incorporated herein by this reference and maintained on file at the Snohomish <br /> County Human Services Department (the "County"), Division of Long Term Care & <br /> Aging ("LTCA"): <br /> A. LTCA's Program Instructions (the "Program Instructions"); <br /> B. Multipurpose Senior Center Guidelines, as now or hereafter amended, <br /> published by the State Aging and Disabilities Services Administration (the <br /> "Guidelines"); and <br /> C. Senior Center Self-Assessment and National Accreditation Manual, published <br /> by National Institute of Senior Centers, 2005 (the "NISC Accreditation <br /> Standards"). <br /> II. REPORTING REQUIREMENTS <br /> The Contractor shall submit required reports, due as provided in the table below, in a <br /> format supplied or approved by LTCA. Overdue reports shall delay payment to the <br /> Contractor until the next billing month. <br /> Report Titles Due <br /> Participant Tracking Report 20th calendar day of the month <br /> following the end of each calendar <br /> month <br /> Quarterly Program Activities Report 20th calendar day of the month <br /> following the end of each calendar <br /> quarter <br /> A.
