k <br /> B. Participant Tracking Reports shall include: <br /> 1. An unduplicated count of participants by current month and year to date; <br /> and <br /> 2. The count shall identify the number of participant residents outside of the <br /> city in which the Contractor's Senior Center (the "Center") is located, by <br /> month and year to date. <br /> C. Program Activities Reports shall include a list of services/programs provided <br /> by the Contractor in the preceding quarter, by category (as set forth in Section <br /> II-1 of Exhibit B). Services/programs must be provided in at least three (3) <br /> different categories. <br /> III. REIMBURSEMENT <br /> A. The Contractor must submit requests for reimbursement no later than ten (10) <br /> calendar days following the end of each calendar month. The request for <br /> reimbursement must be submitted on forms approved by LTCA. The County <br /> shall process correct and properly submitted billings for reimbursement after all <br /> supporting documentation is received. Billings received after the 10th of the <br /> month will be processed for reimbursement in the following month. <br /> B. Monthly billings shall be based on allowable expenses and shall be <br /> accompanied by monthly expenditure reports showing line-item expenditures <br /> corresponding to the Approved Budget or amended Approved Budget <br /> (Exhibit C). <br /> C. The Contractor must submit final request for reimbursement for 2009 expenses <br /> no later than January 10, 2010. Billings received after January 10, 2010 for <br /> expenses incurred in 2009 will not be processed or paid. <br /> IV. TRAINING REQUIREMENTS <br /> The Contractor shall establish a training plan for all employees performing services <br /> under this Contract. The plan shall provide for orientation of new employees and <br /> ongoing in-service training for continuing employees. The training must be provided <br /> by qualified persons- and will include either formal training sessions or on-the-job <br /> training. The dates and topics of training received shall be documented in a central <br /> file or in the personnel files of all employees who have received the training. <br /> V. EMERGENCY PROCEDURES <br /> The Contractor must establish a written plan that describes procedures to be followed <br /> in the event a client becomes ill or is injured while at the Contractor's office or if the <br /> Contractor's staff is in the client's home.
