5 Project: WFP Clearwell No. 2 Change Order No.4 Effective April 06, 2009 Exhibit A - Description of Changed Work Change Order#4 Create New"Bid Item #7 —entitled, "Change Order#4", in the lump sum amount of$55,294.13, which shall incorporate Work Change Directives 043, 046, 051, 052, 055, 056, 057, 058 and 059 per the following: 1. Work Change Directive 043 —Clearwell Flow Distribution Structure construction of an overflow diversion structure and revised location of the stair. Increase Contract amount by $16,293.19. 2. Work Change Directive 046—Backwash and surface wash pump revisions. Increase Contract amount by $4,001.73. Make the following revisions to the backwash pumps and the surface wash pump: A. Add 120 volt circuits to the backwash and surface wash motors, for the space heaters in the motors. B. Reroute the wiring for the limit switch on the Golden Anderson recirculation valve from the existing MCC, in the Clearwell Control Structure, to LCP-BWPS in the new Electrical Room of the Backwash Pump Station. C. Reroute the wiring for the two solenoid valves and relocate a relay, associated with the Golden Anderson recirculation valve, from the existing MCC in the Clearwell Control Structure to LCP-BWPS in the new Electrical Room of the Backwash Pump Station. D. Provide a new 120 volt circuit for the existing level transmitter, LIT-50, from the new 120/240 panel LC, located the new Electrical Room of the Backwash Pump Station. E. Provide new wiring and switches necessary to install a light switch adjacent to door 501 in the Pump Room of the Backwash Pump Station. F. Provide piping for the Golden-Anderson control valve replacing existing supply piping and providing drain piping. 3. Work Change Directive 051 —Provide an oil separator for the tapping tee and Clearwell Flow Distribution Structure excavation dewatering water. Increase Contract amount by $11,401.28. 4. Work Change Directive 052 - Provide temporary power for slide gate CCS-SG-4 located south of the Backwash Pump Station to assist the City of Everett staff during Plant shut down. Increase Contract amount by $ 830.80. 5. Work Change Directive 055 — Install a 4-inch isolation gate valve on the water line to Clearwell No. 2. Increase the Contract amount by$2,355.14. 6. Work Change Directive 056—Add power monitoring devices and suction and discharge gauges to each of the four sample pumps. Increase the Contract by $12,461.91. 7. Work Change Directive 057—Raise an existing chemical vault to the proposed new site grade. Increase the Contract by $3,637.16. 51 Page 3 of 4
