Project: WFP Clearwell No. 2 <br /> Change Order No.6 <br /> Effective August 24,2009 <br /> Exhibit A - Description of Changed Work 6 <br /> Change Order#6 <br /> Create New"Bid Item#9—entitled,"Change Order#6",in the lump sum amount of$169,938.38 to <br /> incorporate Work Change Directives 033,063,064,065,067,068 and 069 per the following: <br /> 1. Work Change Directive 033 —Provided field routing for chemical feed and sample piping that were <br /> not shown on the Contract Drawings and included locations of additional sample and chemical feed <br /> points. Increase Contract by$45,347.65. <br /> 2. Work Change Directive 063 —Construct cast-in-place concrete riser/vault walls to set City of <br /> Everett provided vault lids in two locations. One riser/vault shall be for access to the existing vault <br /> north of the BWPS and the second for access to the fluoride tee and valves at the north side of the <br /> Clearwell No. 1 Control Structure. Increase Contract by$2,719.78 <br /> 3. Work Change Directive 064—Provide a Utility Vault lid No. 77-1660 (7'x7'x6" with a 16"x60" <br /> opening) for the RWPS electrical transformer TRF-UTIL-2 vault. Increase Contract by$504.75 <br /> 4. Work Change Directive 065 —Revise road grades and provide and install railroad ballast to armor <br /> drainage paths east and west of Clearwell No. 2. Increase Contract by$21,115.12. <br /> 5. Change Directive 067-Extend the cast-in-place concrete overflow wall on the south side of the <br /> CFDS to the west 40 LF with ecology blocks. Please remove the top row of the ecology blocks that <br /> were placed around the Flow Element FE-65 and use them when constructing the wall extension <br /> for the CFDS. Install Aluminum Picket Type Guardrail similar to Detail S507/GS-706 on top of <br /> ecology blocks placed at the CFDS and on the remaining ecology block at FE-65. Increase <br /> Contract by$22,903.58. <br /> 6. Work Change Directive 068 —Provide and place 3-inchesof topsoil on Clearwell No. 2 including <br /> the side slopes this is estimated to take approximately 2500 cubic yards of topsoil. Increase <br /> Contract by$74,260.90 and add 14 days. <br /> 7. Work Change Directive 069—Install area inlet and 50 LF of 6-inch pvc pipe.
