3 <br /> EXHIBIT A <br /> SCOPE OF WORK <br /> City of Everett—126th Street SE Drainage Final Design <br /> INTRODUCTION <br /> The open-ditch drainage system along 16th Avenue SE between 126th Street SE and 127th Place SE frequently backs <br /> up with water,resulting in nuisance flooding. In 2008,a Schematic Design Report and 30%Design were completed <br /> that proposed constructing approximately 1,200 LF of 18-inch diameter high-flow bypass pipe along 126th Street SE <br /> between 16`h and 19th Avenues SE. <br /> This Scope of Work identifies services GHD Inc. will perform to prepare the final plans,specifications,and cost <br /> estimate for bidding and construction of the proposed drainage improvements, including design support during <br /> bidding and construction. <br /> PROJECT TASKS <br /> Task 1 Project Management and Administration <br /> 1.1 Maintain ongoing project communications with the City and coordinate completion of the Scope of Work. <br /> 1.2 Provide internal quality assurance/quality control of products. <br /> 1.3 Monitor progress and expenditures to maintain the project schedule and budget. <br /> 1.4 Report monthly in writing to update the City on project progress,expenditures,budget remaining,and any <br /> schedule or budget issues. Each task shall be reported in terms of percent complete and total expenditures. <br /> Deliverables: Project schedule <br /> Monthly invoicesand project status reports <br /> Task 2 Surveying and Utility Location <br /> 2.1 The surveying and Utility Location task will be coordinated by the Consultant,with sub-tasks 2.2—2.7 <br /> performed by survey subconsultant Reid Middleton and sub-task 2.8 performed by Bravo Environmental. <br /> 2.2 Prepare a topographic survey and mapping to City of Everett standards of 126th Street SE between 16th and <br /> 19th Avenues SE from the south pavement edge to the north right-of-way line,ditch tines along 16th Avenue <br /> SE for 200 feet south of 126`h Street SE,and a 40-foot wide strip west of the west gutterline of 19th Avenue <br /> SE between 126th Street SE and 300 feet to the south. <br /> 2.3 Survey mapping will be developed at two-foot contour intervals. Measurement within ground areas will be <br /> taken on an approximate 50-foot grid. The survey will include measurements within the project limits as <br /> follows: <br /> a. At abrupt changes of grade such as slopes,curbs,walls,rockeries,structures,and edge of concrete or <br /> asphalt <br /> b. Existing surface features, visible structures,manholes,catch basins,planters,fences,walkways, <br /> paths,and parking areas <br /> c. Significant trees 6 inches in diameter and larger as measured at breast height and a definition of <br /> generic tree name <br /> d. Signs, light,power and guy poles <br /> Page 1 of 7 19 <br />
