Inc. <br />17515 NE 671h Court <br />Redmond, WA 98052 <br />RE: Clarification of permit information <br />Dear Mr. Monson, <br />I am writing to you in regards to three separate building permit applications you <br />have submitted for wireless telecommunication facilities. The site addresses you <br />have submitted permit applications for are 9610-19r" Ave SE, 1802-751" St SW, <br />and 6320 Evergreen Way. Prior to your submittal for building permits, the <br />Planning Department reviewed the individual proposals through the Special <br />Property Use (SPU) process. Each of the proposals was approved as <br />conditioned in the SPU decision. <br />However, in reviewing your building permit applications, it appears there are <br />some discrepancies in the application materials you previously submitted for <br />review, and the materials you are submitting now. Prior to receiving Planning <br />approval of these building permits, you will need to provide written clarification to <br />the information you are providing. Below is a list of discrepancies that I have <br />noticed; this list may be incomplete, so I would ask that you review all materials <br />at your disposal in order to provide the City with the most accurate information. <br />Information provided for 9610-19" Ave SE: <br />Please provide an accurate site/vicinity map. <br />• Clarify at what height the antennae will be located. <br />Information provided for 1802-751h St. SW: <br />• Project description is written as collocation on a six story <br />building, when it is actually collocation on an existing monopole. <br />• Sketch provided for SPU review showed a 160' monopole; <br />building permit sketch shows a 99' monopole.
