PERMIT # SW -204 <br />PERMIT PERIOD: JULY 1, 2009 TO JUNE 30, 2010 <br />Page 3 of 7 <br />SECTION II: PERFORMANCE STANDARDS <br />The owner or operator shall: <br />A. Design, construct, operate, and close all facilities in a manner that does not pose a threat to <br />human health or the environment; <br />B. Comply with Chapter 90.48 RCW, Water Pollution Control and implementing regulations, <br />including Chapter 173-200 WAC, Water Quality Standards for Ground Waters of the State of <br />Washington; <br />C. Conform to the approved local comprehensive solid waste management plan prepared in <br />accordance with Chapter 70.95 RCW, Solid Waste 11anagement — Reduction and Recycling, <br />and/or the local hazardous waste management plan prepared in accordance with Chapter <br />70.105 RCW, Hazardous Waste Management; <br />D. Not cause any violation of emission standards or ambient air quality standards at the property <br />boundary of any facility, and comply with Chapter 70.94 RCW, Washington Clean Air Act; <br />and <br />E. Shall comply with all other applicable local, state, and federal laws and regulations. <br />F. The Health District requires that the landfill operate with an approved reclamation permit <br />from the Washington State Department of Natural Resources (DNR). If the permit is denied <br />for any reason the Health District permit is subject to suspension. <br />If the performance standards of this section are not met, corrective actions (approved by the <br />Health District) shall be designed and implemented, and enforced on a time schedule approved <br />by the Health District. <br />SEC'T'ION III: SPECIFIC CONDITIONS <br />A. The Cemex Incorporated (Cemex Inc.) Inert Landfill shall operate in accordance with the <br />approved plan of operation dated March 25, 2005, and any approved amendments thereafter. <br />The permittee shall notify the Health District in writing prior to any deviation from or change <br />in the operating plan. These changes will require Health District approval prior to <br />implementation. <br />B. On entering the premises, customers using the landfill shall be directed to the main scale <br />house. Customers will be required to fill out a ticket indicating the hauler and the source, <br />type, and quantity of waste. <br />C. Scale masters shall conduct a visual check of each incoming load, and shall turn away any <br />load containing unacceptable material. In addition the landfill operator (dump man) will <br />visually inspect dumped loads for unacceptable material_ In the event that unacceptable <br />material is identified, the waste shall be segregated to a separate stockpile area and remove <br />with twenty-four (24) hours. <br />
