• <br /> EIS be prepared because of the probable <br /> significant adverse environmental impact <br /> described in the application; <br /> d. The applicant requests an extension (not to <br /> exceed an additional thirty (30) days) ; or <br /> e. If the applicant revises the application and <br /> such revision requires recirculation or <br /> additional analysis, the ninety (90) day <br /> period shall commence upon submittal of the <br /> revised application. <br /> Any time limits set forth in this subsection shall not <br /> apply to withdrawal of affirmative and negative <br /> threshold determinations (DS, DNS) where such <br /> withdrawals are made in accordance with WAC 197-11-340 <br /> and 197-11-360; and <br /> For purposes of the ninety (90) day period, an <br /> application and supporting documentation is deemed <br /> complete at such time as the Responsible Official <br /> issues a Certification of Completion. Such <br /> Certification will be issued in accordance with an <br /> administrative rule adopted by the Responsible <br /> Official. Upon issuance of a Certification of <br /> Completion, the Certification shall only be withdrawn <br /> in the following circumstances: <br /> 1. There are substantial changes to a proposal so <br /> that the proposal is likely to have significant <br /> adverse environmental impacts; <br /> 2 . There is significant new information indicating, <br /> or on, a proposal ' s probable significant adverse <br /> environmental impacts; or <br /> 3 . The Certification of Completion was procured by <br /> misrepresentation or lack of material disclosure. <br /> In the event that a Certificate of Completion is withdrawn <br /> and the Responsible Official determines that additional <br /> information is needed to process the application, the <br /> applicant shall be so notified, and the ninety (90) day <br /> period stayed pending receipt of the requested information <br /> by the City. <br /> 7 <br />
