4. <br /> ATTACHEMENT 6 <br /> "City Of Everett—Smart MOVE" <br /> Guaranteed Ride Home(GRH) <br /> In order to use this service, an employee must be an official participant of the rideshare <br /> program. This service may be used only on those days when an employee car/vanpools, bikes, <br /> walks, or has ridden the bus to work and must unexpectedly leave the worksite.The policy does <br /> not apply when the employee drives alone to work. The intent of the policy is to insure you a <br /> ride to your car or home in the case of an emergency. Transportation will be provided within a <br /> 50-mile radius of your place of work. An emergency situation consists of one of the following: <br /> A. Sudden illness/injury to family members, yourself, or pet. <br /> B. Crisis at home, such as flooding, etc... <br /> C. Other cases as determined on an individual basis. <br /> To use the GRH service, please follow these procedures: <br /> A. Tell your immediate supervisor of the emergency. <br /> B. Upon verifying that it is a"covered"emergency, your supervisor will call the ETC <br /> (Employee Transportation Coordinator)to determine the best way to handle the <br /> situation. You will usually be transported by City vehicle or taxi <br /> C. The ETC will make arrangements and call you back within 5 minutes to explain your <br /> ride home. <br /> D. It the ETC is unreachable for any reason, your supervisor will be authorized to make <br /> arrangements and to notify the ETC of those arrangements. <br /> E. If you require transportation to a hospital, subsequent transportation to your car or <br /> home will be your responsibility. <br /> F. Each registered participant is eligible for 6 trips per calendar year. <br /> The same travel options are also available to a participant who is unexpectedly asked by his/her <br /> supervisor to work overtime, thereby missing a scheduled Rideshare departure time. <br /> ORCA Passport Agreement Page 23 of 23 City of Everett BA ID:1891 <br /> December 1,2010-November 30,2011 <br /> 34 <br />
