Exhibit A to the Professional Services Agreement <br /> between The City of Everett and The Blueline Group,LLC for the <br /> Water"L"Improvements- Hewitt Ave(WO#UP3462) <br /> dated October 28,2011 <br /> PROJECT DESCRIPTION <br /> The City of Everett plans to replace approximately 2,200 lineal feet of existing 12-inch cast iron water <br /> main with 12-inch ductile iron water main along the south side of Hewitt Avenue from: <br /> Under the 1-5 Ramp Bridge just east of Maple Street westerly to the easterly edge of <br /> the bridge at the railroad tracks approximately 250-300 feet west of Virginia Avenue. <br /> The Blueline Group, LLC("Blueline") will provide engineering services to the City of Everett("City")for <br /> this project, known as Water "L" Improvements - Hewitt Ave ("Project"). <br /> As outlined herein, Blueline will provide topographic base mapping, limited geotechnical <br /> explorations, design drawings, specifications, engineer's estimates (PS&E), and limited bidding and <br /> construction services. Design will consist of water main, fire hydrants, connections to existing mains, <br /> water services, other appurtenances, and surface restoration. <br /> PROJECT SCHEDULE <br /> Blueline shall begin work immediately upon receipt of Notice to Proceed from the City and proceed <br /> according to the following approximate Project Schedule. Key dates include: <br /> Notice to Proceed Early November 2011 <br /> Survey& Base Mapping November/ December 2011 <br /> Geotechnical Explorations November/ December 2011 <br /> Preliminary Design Submittal January 2012 <br /> 60% Design Submittal February 2012 <br /> 90% Design Submittal March 2012 <br /> Final Design Documents April 2012 <br /> Bidding&Award April/ May 2012 <br /> Construction Begins June 2012 <br /> Construction Ends September 2012 <br /> As-Builts& Project Close-out October 2012 <br /> SCOPE OF WORK <br /> Blueline's scope of work for the Project is detailed in the following pages. <br />
