(b) Outcome 2: Clients will meet a majority of their self-sufficiency goals (e.g., housing, education, <br /> employment) as outlined in the client's Independent Living Plan (ILP). <br /> Each client who engages in the program works with the Case Manager to create an ILP with <br /> specific self-sufficiency goals that focus on housing, education and employment related <br /> milestones. The Case Manager meets with each client weekly to assess their progress and <br /> provide support; they also review ILP goals every three months while the client is in the program. <br /> Progress toward goals can include development of various life skills such as financial <br /> management, basic household management skills, employability and job search, and educational <br /> activities; for example, creating or improving a resume, going to an interview, participating in job <br /> training or starting GED courses. Our target for this outcome is 90% and we have consistently <br /> met this outcome with 100%, 94% and 100% in 2014, 2015, and 2016 respectively. <br /> (7) Methods and Tools Used to Evaluate Progress: <br /> Friends of Youth utilizes an agency-wide client information database, called Youthforce, to track all <br /> client information and progress. Program staff enter case notes, assessment results, community <br /> referrals, outcome progress, independent living plan goal progress, demographics and aftercare <br /> follow-up directly into Youthforce. Our data management specialist works with program staff to ensure <br /> data quality, tracks outcomes and trends, and compiles statistics for reporting to our government <br /> funders at the city, county, state and federal levels. We directly enter client information into the <br /> Snohomish County HMIS, and the U.S. Department of Health and Human Services, Runaway and <br /> Homeless Youth data management system, RHYMIS. Data is entered into these systems on a <br /> regular schedule with consistent data quality checks. <br />
