Afi <br /> tREQUEST FOR PROPOSALS <br /> Ws General Contractor/Construction Manager Services for <br /> Water Pollution Control Facility Expansion-Phase C <br /> 2. List three (3) past or current projects performed by each of the key <br /> personnel including the following: <br /> a. Project name, location, and brief description of project <br /> including its size, scope, and complexity. <br /> b. Description of the individual's position and responsibilities on <br /> the projects. <br /> c. Whether the individual worked for the contractor, <br /> subcontractor, or other. <br /> d. Relevance of the project experience to the work required for <br /> this Project, including project delivery methods, such as <br /> GC/CM or similar contracting methods. <br /> e. Contact information for each project including the owner and <br /> firm employing the key individual. References may be <br /> contacted by the City of Everett and used as part of the <br /> evaluation process. <br /> 3. Describe the extent of involvement of the individuals assigned during <br /> each phase of the Project (preconstruction, subcontract buyout and <br /> construction) expressed as a percentage (100% = full time). <br /> 4. Provide a copy of resumes for all key primary and backup personnel <br /> with the history of employment, education, work experience, length of <br /> time with the firm, and any other information the selection committee <br /> might find useful in evaluating the qualifications and abilities of the <br /> individuals proposed. <br /> B. Qualifications and Experience Required for Key Personnel: Demonstrate <br /> how the key primary and backup personnel proposed in A above meet the <br /> following qualifications (qualifications provided through other team members <br /> is not sufficient). <br /> Project Manager <br /> 1. Experience with developing publicly advertised and competitive <br /> subcontract and supply bid package and managing multiple <br /> subcontractors <br /> 2. Experience as a lead project manager on a GC/CM, CM/GC, and/or <br /> CM@Risk <br /> 3.
