Project: WPCF Phase B-2 <br /> 7 Change Order No.5 <br /> Change Order Effective Date: April 9, 2012 <br /> •CONTRACTOR,t z; ,- ' 7).! } � , <br /> By Date: f h 7,0 t2-- <br /> Officer <br /> Exhibit A—Description of Changed Work <br /> Add Bid Item Change Order#5 adds one (1) new bid item to the contract as listed below. <br /> Change Order#5,Schedule A Work: <br /> Create New "Bid Item #A13—entitled, "Force Account for Sluice Gate Installation" in the amount of <br /> $250,000.00, which will incorporate all labor, equipment, materials, and markups to complete the <br /> installation of two (2) Stainless Steel 54-Inch Sluice Gates, as shown below. <br /> Work Change Directive WCD-012 "' In reference to the 54-Inch gates G-001 and G-002 located in AC-2 <br /> and shown in Detail 1 on Drawing C-3. Purchase and install (2ea) new stainless steel gates, <br /> Limitorque gate operators, stainless steel frames and pipe inserts per approved submittal #59A. This <br /> work includes divers for underwater work and any necessary excavation in AC-2 to allow for installing <br /> the gates. Work to be completed in accordance with the Section 9-Measurement and Payment, <br /> Subsection 9.6-Force Account, Pages 00715-00718 of the General Conditions for this project. <br /> JUSTIFICATION: <br /> Work Change Directive WCD-012 ^'Gates G-001 and G-002 are needed for operational control <br /> between the second Aeration Cell (AC-2) and the Oxidation Pond. They must be closed to allow level <br /> control of the Aeration Cells with the AC-2 Level Control Structure and to isolate AC-2 from the <br /> Oxidation Pond. Operating the Aeration Cells at a higher level than the Oxidation Pond preserves <br /> maximum storm water storage capacity in the Oxidation Pond and prevents back-flow of water from <br /> the Oxidation Pond to AC-2. They must be opened to provide maximum flow out of aeration cells <br /> during storm events and prevent damage to the aeration cell dikes from over-filling. <br /> The existing gates were discovered to be severely deteriorated and beyond repair during the contract <br /> specified work of slip lining the pipe between AC-2 and the Oxidation Pond. <br /> CONTRACT TIME: <br /> There are 108 calendar days (January 14, 2012-April 30, 2012) added to the Schedule A Work by this <br /> change order#5. <br /> 72 <br />
