EXHIBIT A <br /> SCOPE OF WORK <br /> The purpose of this contract is to provide asbestos survey services for the Evergreen Branch Library Expantion <br /> Project located at 9512 Evergreen Way,Everett, Washington. The approximate size of the addition is anticipated to <br /> be 5,552 square feet based on a previous feasibility study,with 36 parking stalls added to the site. <br /> The scope of services for this project is to conduct the environmental surveys to provide information for worker <br /> protection during maintenance,abatement,renovations and construction activities at the site. Additionally, service <br /> provider will incorporate completed city owned Duplex asbestos survey(provided by City representitive)into final <br /> survey documents. <br /> The following services include: <br /> 1. Hazardous Material Surveys <br /> a. Asbestos: <br /> 1. Design document review. <br /> 2. Asbestos Survey. <br /> 3. Sampling and Analysis-30 PLM Samples and 5 Samples utilizing 400 point count process. <br /> b. Lead: <br /> 1. Lead paint sampling and analysis. <br /> 2. Water sampling and analysis. <br /> 3. Sampling and analysis for 15 Samples <br /> c. Hazardous Chemicals Hg and PCB" <br /> 1. Fluoresent Light Fixtures-Mercury(Hg)and ballest Polychlorinated Biphenyls(PCB) <br /> 2. Alternative Environmental Technologies(AET)Services: <br /> a. Asbestos Good Faith Inspection(AGFI)and Certified Report. <br /> b. Lead Survey included with AGFI. <br /> c. Light fixture components inventory with AGFI report. <br /> 3. Services include the required Asbestos Good Faith Inspection(AGFI), including a lead survey and light fixture <br /> inventory so that the hazardous materials: asbestos,lead,mercury and PCBs,can be identified for worker protection <br /> during any work activity that may disturb these materials,abatement prior to renovation,and new contruction <br /> proposed for subject site. <br /> Page 12 <br /> (Form Approved by City Attorney's Office January 7,2010,updated June 15,2014) <br />
