The face amount of life insurance and the principal sum of accidental <br /> death and dismemberment insurance for an Aappointive°ofcer shall be <br /> equal to two times the Appointive Officer's basic annual earnings, <br /> rounded to the next higher multiple of$1,000,if not already a multiple of <br /> $1,000,plus$2,000. The maximum face amount and maximum principal <br /> sum shall each be$150,000. <br /> 4. Repealed by Ordinance No. 2560 01. <br /> 35-. Long -term disability insurance shall be 66 2/3% of the first$7,500 of the <br /> Appointive Officer's pre-disability monthly earnings,subject to policy <br /> conditions and limitations. <br /> 46. VoluntaryOptional dependent life insurance covered at the Appointive <br /> Officer's expense. <br /> 57. Annual physical examinations for all Department Heads. <br /> 8.
