Levels of Accomplishment- Goals and Performance Measures <br /> Domestic Violence Services will convert the Oswald Army Reserve property into a <br /> 52-bed shelter, administrative offices, supportive services, and commercial <br /> kitchen. In the first year of operation the new Domestic Violence will serve at <br /> least 52 unduplicated citizens from Everett who has been victims of domestic <br /> violence. <br /> The Subgrantee agrees to track outcome performance measures as required by the U.S. <br /> Department of Housing and Urban Development and the City. The Subgrantee will report the <br /> outcomes to the City on at least a quarterly basis. Information and forms will be provided to the <br /> Subgrantee by the City for reporting this information. <br /> E. Time of Performance <br /> Services of the Subgrantee shall start on the 6th day of April 2011 and end on the 30th day of June <br /> 2013. The term of this Agreement and the provisions herein shall be extended to cover any <br /> additional time period during which the Subgrantee remains in control of CDBG funds or other <br /> assets, including program income. The City may, at its discretion, extend the term of this Agreement <br /> to allow for the expenditure of unexpended funds. <br /> The funding has come from two program years. $172,821.67 (2011) and $100,000.00 (2012) <br /> Predevelopment costs can be charged starting with expenses from April 6, 2011 on. When bills are <br /> submitted they need to be by type of cost and during what time frame. The time frames are: <br /> April 6, 2011 to June 30, 2011 <br /> July 1, 2011 to June 30, 2012 and <br /> July 1, 2012 to June 30, 2013. <br /> A-3 <br />
