City of Everett Human Needs Grant <br /> January 1, 2017 through December 31, 2017 <br /> Exhibit B <br /> Terms, Conditions and Projected Budget <br /> Organization: Work Opportunities <br /> Program: Employment Supports for Individuals with Disabilities <br /> Will spend the funds received for the specific purposes of: <br /> Our program services support an individual along their pathway to employment by providing a continuum of <br /> activities to help individuals identify abilities and interests, develop skills necessary for successful employment, <br /> and find and maintain employment. We also provide supported employment services, a unique approach that <br /> helps individuals with disabilities identify a career goal, find employment, and receive on the job training. Long <br /> term follow along support is crucial for successful, continued employment for individuals with the most complex <br /> support needs. Our program services provide persons with disabilities with employment and/or the tools and <br /> skills necessary to obtain and maintain employment; and by doing so, maximizing their independence and their <br /> ability to participate fully in the mainstream of community life. <br /> For the purposes of this grant, a unit of service is defined as: <br /> One Hour <br /> Work Opportunities <br /> anticipates this grant will fund a total of 560 units of service for (unduplicated count) 9 Everett <br /> residents each quarter (define period — month, quarter, etc.) <br /> Work Opportunities <br /> anticipates its budget for the year 1/1/2016 through 12/31/2016, by quarters, will be: <br /> REVISED BUDGET First Qtr.
