City of Everett Human Needs Grant <br /> January 1 through December 31,2017 <br /> Housing Hope <br /> Child and Family Specialist Program <br /> EXHIBIT A: Program Summary <br /> Program Objective: <br /> • Assist families to increase parenting skills to decrease abuse and neglect <br /> • Provide one-on-one support and guidance around a child's education and future <br /> • Connect parents to appropriate services for their children's developmental needs <br /> • Connect teens to mental health and chemical dependency service <br /> • Coordinate with the Everett School District about Individualized Education Programs(IEP's),developmental <br /> delays and truancy issues <br /> Program Components and Structure: <br /> Homelessness and poverty can be extremely traumatic for developing children.The Child and Family-Speci lists'prdvide. <br /> vital,in-home services to the children and their parents to overcome these traumas and set families on the path for a <br /> lifetime of success. Services are varied and tailored to the individuals needs of the child and family. <br /> General services include: <br /> • Assessing the existing situation <br /> • Establishing goals for improvement <br /> • Connecting the child and parent(s)to community resources to mitigate the issue <br /> • Follow-up to ensure and document the improved situation. <br /> Specific examples of intervention for children include: <br /> • For developmental delays in children aged birth to 5:assess the delay and make referrals to the ChildStrive <br /> Parents-As-Teachers program. Design intervention services for parents and classroom teachers. <br /> • For medical,dental and mental health issues:assess and refer to partners such as Providence General, <br /> Children's Hospital,Sea Mar Community Health Center,Catholic Community Services or Compass Mental <br /> Health. <br /> • Many homeless or low income children are on the autism spectrum. Housing Hope works with the Sensory <br /> Processing Disorder Program at Providence General and supports parents to complete the often daunting <br /> application process. <br /> • Before CPS involvement:Specialists can provide intensive services working on issues of safety in the home to <br /> keep the situation from escalating to a point where CPS must be called. <br /> • In cases of child abuse or neglect: Ramp up intensive services as the child transitions either out of or back into <br /> the home. <br /> • Work one-on-one with older children and school attendance if needed.Assist in developing a morning routine <br /> to attend school and to see the importance of their education and future. <br /> • Connecting teens to counseling for emotional disturbances, including eating disorders and suicidal <br /> thoughts/actions. <br /> • Promoting Alternative Thinking Strategies(PATHS)social/emotional developmental growth group for school <br /> age children. <br />
