ARTICLE 12 - INSURANCE BENEFITS <br /> Section 1: Eligibility. <br /> a. New hire employees shall not receive any City paid insurance benefits until the <br /> first day of the 3rd month of employment (employee starts work in April, <br /> insurance starts June 1St) <br /> b. Regular part-time employees who work 22 hours or more per week shall be <br /> eligible for benefits under Article 12 on a pro-rata basis. <br /> c. Day laborers (IV) are exempt from Article 12 unless required by law. <br /> d. Library Pages are exempt from Article 12 unless required by law. <br /> Section 2: Medical Insurance. <br /> The City agrees to offer medical insurance that is acceptable to the City for employees and <br /> their eligible dependents who request coverage, provided the employee makes a ten percent <br /> (10%) premium contribution. <br /> The benefits under the City's self-insured medical insurance program are described in Appendix <br /> 2, and such benefits shall not be reduced without agreement of the parties. <br /> If an employee chooses a carrier other than the City's self-insured medical insurance program, <br /> the employee shall pay the premium difference. <br /> Section 3: Life Insurance. The City will provide for the employee $1,000 life insurance per <br /> $1,000 of annual salary, rounded to the next highest $1,000 at the City's expense for the <br /> duration of this agreement. <br /> Section 4: AD&D. The City will provide for the employee Accidental Death and <br /> Dismemberment coverage in an amount equal to that specified in Section 3 hereinabove, at the <br /> City's expense for the duration of this agreement. <br /> Section 5: Dental. The City agrees to provide one hundred percent (100%) of the premium <br /> cost toward the purchase of a dental insurance plan that is acceptable to the City for employees <br /> and their eligible dependents. Effective January 1, 2006 the annual maximum shall be <br /> increased to $2,000. <br /> Section 6: Vision. The City will pay the premium for the vision plan offered through the <br /> trustees of the Washington State Council of County and City Employees for the employee and <br /> his/her eligible dependents. <br /> Section 7: Enrollment. In order to participate, an employee must sign authorization cards to <br /> be kept on file by the City. If the employee does not wish to participate in the program, then <br /> he/she must sign a waiver card for his/her file. <br /> Section 8: Review. The City will provide reports on the City's self-insured plan three times a <br /> year which will include usage trends and fund balance. Prior to setting the premiums each <br /> Page 28 <br />
