INSTRUCTIONS TO BIDDERS <br /> 1.0 Design Engineer <br /> Questions and inquiries about these Specifications and Contract Plans should be directed to the <br /> attention of Narci Ricafrente, (425) 257—7285 or nricafrente@everettwa.gov. <br /> 2.0 Bidder's Check List <br /> The bidder's attention is directed to the following City-provided forms which must be executed in <br /> full as required and submitted with the bid: <br /> 1. Proposal: The lump sum and unit price items must be shown in the space provided. Show <br /> unit prices in figures. <br /> 2. Proposal Signature Sheet: To be filled in and signed by the Bidder. <br /> 3. Subcontractor List: To be filled in by the Bidder. <br /> 4. Disadvantaged/Women's Business Enterprise Contact Form: To be filled in and signed <br /> by the bidder. <br /> 5. Non-Collusion Declaration: To be submitted with the bid. <br /> 6. Bid Bond: This form is to be executed by the Bidder and the surety company unless bid is <br /> accompanied by a certified check. The amount of this bond shall be not less than five percent <br /> (5%) of the total amount bid and may be shown in dollars or on a percentage basis. Cash will <br /> not be accepted. <br /> Failure to complete the aforementioned forms and to submit said forms with the bid may be due <br /> cause for rejection of bid. <br /> 3.0 Contract Forms <br /> The following forms are to be executed after the award of Contract: <br /> 1. Contract: This Contract to be executed by the successful bidder in duplicate within twenty <br /> (20) calendar days after the award date. <br /> 2. Performance Bond (DOT Form 272-002A): This form is to be executed by the successful <br /> bidder and his surety company in triplicate. The amount of this bond shall be one hundred <br /> percent (100%) of the amount of the bid and shall be submitted with the contract. <br /> 3. Payment Bond (DOT Form 272-003A): This form is to be executed by the successful bidder <br /> and his surety company in triplicate. The amount of this bond shall be one hundred percent <br /> (100%) of the amount of the bid and shall be submitted with the contract. <br /> 4.
