Failure to comply with this requirement may render the <br /> Bid non-responsive and may cause its rejection. This Addendum consists of 39 pages, including <br /> all revisions, attachments and details. <br /> The Bid date, time, and location for receipt of Bids are unchanged by this Addendum. <br /> 2:00 PM MAY 16, 2017 AT THE CITY CLERK'S OFFICE, 2930 WETMORE AVENUE. <br /> THE CONTRACT DOCUMENTS ARE MODIFIED AS FOLLOWS: <br /> SPECIAL PROVISIONS <br /> 1. Replace the Bid Proposal form with the attached replacements for page P3 <br /> through P12. The Bid List has been revised as follows: <br /> a.
