PUBLIC WORKS <br /> < rJErr <br /> ADDENDUM NO. 3 <br /> GRAND AVENUE PARK BRIDGE <br /> W.O. #3588 <br /> MAY 2, 2017 <br /> NOTICE TO PLANHOLDERS: <br /> Addendum No. 3 contains the following revisions, additions, deletions and clarifications and is <br /> hereby made a part of the contract documents for the above named Project and shall be <br /> considered by all prospective bidders in submitting bids. <br /> Bidders must acknowledge receipt of Addendum No. 3 in the space provided on page P-14 in <br /> Division P, Proposal Signature Sheet. Failure to comply with this requirement may render the <br /> Bid non-responsive and may cause its rejection. This Addendum consists of 12 pages, including <br /> all revisions, attachments and details. <br /> The Bid date, time, and location for receipt of Bids are unchanged by this Addendum. <br /> 2:00 PM MAY 16, 2017 AT THE CITY CLERK'S OFFICE, 2930 WETMORE AVENUE. <br /> THE CONTRACT DOCUMENTS ARE MODIFIED AS FOLLOWS: <br /> BID PROPOSAL <br /> 1. Replace the Bid Proposal form with the attached replacements for page P9. <br /> The Bid List has been revised as follows: <br /> a. Bid Item No. A162, the quantity is revised from "100"to"125". <br /> b. Bid Item No. A163, the quantity is revised from"1"to"2". <br /> c. Bid Item No. A167, the quantity is revised from"400"to"500". <br /> d. Bid Item No. A170, the quantity is revised from"3"to"2". <br /> CONTRACT PLANS <br /> 1. Replace Sheet No. 60 with attached. Drawing A6.1 is revised to show the correct <br /> framing for the canopy. <br /> 2. Replace Sheets No. 192 through 199 with attached. Various details and notes on <br /> the following drawings have been revised to delete stainless steel requirement for bridge <br /> mounted conduit. <br /> a. Sheet No 192 (Drawing E1.01) <br /> b.
