Project: 41st St to WMVD Freight Corridor Improvements <br /> Change Order No.2 <br /> Change Order Effective Date: June 15, 2017 <br /> Total Cost for Modified Bid Items: $130,800.00 <br /> Total Cost of Change Order # 2: $212,714.50 <br /> JUSTIFICATION <br /> Added Bid Items <br /> Bid Item #77—Equitable Adjustment for Delays. Utility firms had not yet relocated their facilities <br /> prior to construction of this project. This bid item is being added to account for delays, caused by <br /> utility relocation work, which prevented the Contractor from completing bid item work in a timely and <br /> efficient manner. <br /> Bid Item # 78—Vehicle Loop Detector Installation. Vehicle detector loops were damaged on <br /> Pacific Avenue following bituminous road planing work but were not shown on plans as being <br /> replaced. This bid item is being added to account for costs to pay for additional loop installation. <br /> Bid Item # 79—HMA Class '/Z Inch, PG 64-22 Pavement Patch. The Contract plans and <br /> specifications did not include a bid item for HMA Pavement Patch. This bid item is being added to <br /> account for costs to patch roadway with HMA Class '/2 Inch, PG 64-22 Pavement Patch. <br /> Bid Item # 80—Cement Concrete Extruded Curb. The Contract required cement concrete extruded <br /> curb to be installed as shown on the plans but there was no corresponding bid item to pay for the work. <br /> This bid item is being added to account for costs to install cement concrete extruded curb. <br /> Modified Bid Items <br /> Bid Item#21 —Commercial HMA. This bid item overran and is being modified to account for costs <br /> to pay for additional Commercial HMA. <br /> Bid Item #37—Flaggers and Spotters. This bid item overran and is being modified to account for <br /> costs to pay for additional flagging and spotting quantities. <br /> Contract Time: Fifteen (15)working days are added to the Contract time as a result of <br /> Change Order#2 as detailed below. <br /> A. Ten (10)working days are added to account for time to rebuild sections of road on Pacific <br /> and on Rucker. <br /> B. Five (5)working days are added to account for time to replace damaged vehicle detector <br /> loops and to install additional vehicle detector loops. <br /> C. Additional working days to be added at a later date to account for equitable adjustment <br /> to Contract. <br /> Page 4 of 4 <br />
