1 <br /> CITY OF EVERETT PUBLIC WORKS 00 22 13 <br /> SPECIAL PROVISIONS SUPPLEMENTAL INSTRUCTIONS <br /> TO BIDDERS <br /> DOCUMENT 00 22 13—SUPPLEMENTAL INSTRUCTIONS TO BIDDERS <br /> ' 1.1 GENERAL <br /> ' A. It is the intent of City to award a contract to the lowest responsive and responsible Bidder. <br /> Before award, the Bidder must also meet the supplemental bidder responsibility criteria listed <br /> below. <br /> B. Further,the apparent low Bidder and second apparent low bidder must submit the Bidder <br /> Qualification Statement (DOCUMENT 00 45 13) and any other documentation listed <br /> below to the City within one business day after the bid opening, unless the City's project <br /> manager,in writing,allows additional time. <br /> C. The City reserves the right to require such documentation from other bidders also. <br /> 1.2 SUPPLEMENTAL RESPONSIBILITY CRITERIA <br /> A. Experience <br /> 1. Criteria: <br /> a. The Bidder must have successfully completed at least two Similar Projects within <br /> the ten years prior to bid opening. <br /> b. The Bidder's superintendent for the Project must have successfully supervised, or <br /> been in a supervisory capacity such as superintendent or site foreman, to <br /> i completion at least one Similar Projects within the ten years prior to bid opening. <br /> 2. Definition of Similar Project: <br /> a. A"Similar Project"means a project that meets all of the following requirements: <br /> 1) Awarded total contract value of at least$300,000 dollars. <br /> 2) Having succesfully installed at least 1,000 linear feet of 24-inch diameter or <br /> greater welded steel pipeline. <br /> 3. Documentation: <br /> a. The Bidder shall submit the Bidder Qualification Statement as provided in <br /> Document 00 45 13. <br /> ' b. The City may also use independent sources of information that may be available to <br /> demonstrate whether the Bidder is in compliance with these criteria. <br /> 4. Evaluation: <br /> a. In evaluating whether these criteria are met,the City may check references for the <br /> previous projects and may evaluate the project owner's assessment of the Bidder's <br /> performance,including but not limited to the following areas: <br /> 1) Quality control; <br /> 2) Safety record; <br /> 3) Timeliness of performance; <br /> ' 4) Use of skilled personnel; <br /> 5) Management of subcontractors; <br /> 6) Availability of and use of appropriate equipment; <br /> THREE LAKES VALVE BYPASS July 19, 2017 <br /> WO#UP3611 00 22 13 - 1 <br />
