3.4 Vendor: Third-party administrator of Everett Low-income Assistance <br /> Program <br /> 3.5 Utility--City of Everett Water, Sewer, and Stormwater Utility - Fund 401 <br /> 4.0 Procedures <br /> 4.1 Eligibility <br /> 4.1.1 Applicant must be served by an individual single-family water <br /> connection. <br /> 4.1.2 Water service to applicant must be directly provided by the Utility <br /> 4.1.3 Location or residence must be within the Everett City limits. <br /> 4.1.4 Applicant that receives a rate discount under the low- <br /> income/disabled senior rate discount program is not eligible for <br /> assistance through the Program. <br /> 4.1.5 Applicant must have received a yellow door hanger notifying <br /> customer of impending water service disconnection. <br /> 4.1.6 Applicants' household income shall not exceed 125% of current <br /> Federal Poverty Guidelines. <br /> 4.1.7 Applicant must provide reasonable proof of financial qualification for <br /> the program. <br /> 4.1.7.1 Proof of income may consist of the following items: <br /> 4.1.7.1.1 Most recent tax return <br /> 4.1.7.1.2 Most recent employment paycheck stub <br /> 4.1.7.1.3 DSHS award letter <br /> 4.1.7.1.4 Child support statement <br /> 4.1.7.1.5 Employment Security statement <br /> 4.1.7.1.6 Other proof of income may be considered <br /> 4.1.8 Applicant must be able to provide proof of household residents. <br /> Proof may consist of the most recent tax return showing <br /> dependents or other documentation deemed reliable by City staff or <br /> Vendor. <br /> 4.2 Availabilty <br /> 4.2.1 Applicants are eligible for up to $200 per year in assistance towards <br /> payment of water and sewer bills. <br /> 2 <br />
