2. The face amount of life insurance and the principal sum of accidental <br /> death and dismemberment insurance for a Councilmember shall be equal <br /> to two times the Councilmember's basic annual earnings, rounded to the <br /> next higher multiple of$1,000, if not already a multiple of$1,000,plus <br /> $2,000. The maximum face amount and maximum principal sum shall <br /> each be $150,000. <br /> 3. Long-term disability insurance shall be 66 2/3% of the first$3,000 of the <br /> Councilmember's pre-disability monthly earnings, subject to policy <br /> conditions and limitations. <br /> 4. Voluntary dependent life insurance covered at the Councilmember's <br /> expense. <br /> is hereby amended to read as follows: <br /> A. Members of the City Council of the City of Everett will be offered <br /> insurance plans comprised of the following on terms and conditions <br /> determined by the City: <br /> 1. Basic and major medical, dental, and vision coverage for the <br /> Councilmember and eligible dependents, provided the Councilmember <br /> makes a 10 percent contribution toward the cost of the medical coverage. <br /> If a Councilmember chooses a carrier other than the City's self-insured <br /> medical insurance program,the Councilmember shall pay the difference in <br /> premiums. <br /> 2.
