D. Levels of Accomplishment - Goals and Performance Measures <br /> In addition to the normal administrative services required as part of this Agreement, the Subgrantee <br /> agrees to provide the following levels of program services: <br /> Activity Units of Service* per Month Total Units/Year <br /> Parent/Youth Family 30 Seminar Hours 360 hours <br /> Seminar <br /> Total People or Households to be served Total unduplicated Everett Residents or <br /> Households to be served <br /> 110 households 110 households <br /> *Note: Provide definition of Units of Service, which may include such measures as units rehabbed, <br /> persons or households assisted, or meals served, here. If this is a construction project, please <br /> explain here how the improvements will be used and how they benefit the community, what the <br /> target population is for the project, etc. <br /> The Subgrantee agrees to track outcome performance measures as required by the U.S. <br /> Department of Housing and Urban Development and the City. The Subgrantee will report the <br /> outcomes to the City on at least a quarterly basis. Information and forms will be provided to the <br /> Subgrantee by the City for reporting this information. <br /> E. Staffing <br /> [Add Provide a list of staff and time commitments to be allocated to the activity specified in I.A. above] <br /> Interim Chief Executive Officer: Oversees all agency programs and operations including supervision of <br /> the Chief Financial Officer, Chief Philanthropy Officer, and the Directors Development and Community <br /> Relations, Programs, and Human Resources. Requires a bachelor degree with 5 years of relevant <br /> experience. <br /> Director of Prevention: Oversees all Prevention programs, including WayOUT, providing practical and <br /> clinical oversight to ensure that Prevention programs work toward the agency's mission and that program <br /> participants make consistent progress towards outcomes. This position requires a Bachelor's degree <br /> in psychology, sociology, or non-profit management. <br /> WayOUT Manager: Operates the program and provides instruction for the WayOUT seminars. The <br /> manager is responsible for ensuring fidelity to the program model while providing a welcoming,supportive, <br /> and culturally appropriate learning environment.This position requires a Bachelor's degree in psychology, <br /> sociology,or social work with experience working with youth and parents of varied backgrounds, and <br /> demonstrated skills in facilitation, instruction, adolescent development, and engaging adult learners <br /> Additional Staff/Community Educators/Partners: Other program staff and community partners provide <br /> instruction modules for the WayOUT program and include mental health providers, law enforcement, <br /> A-3 <br />
