• <br /> 1 Section 9: Application-Procedures-General . <br /> 2 9.1 No license required by this chapter shall be issued unless <br /> 3 written application has been made with the licensing authority. The <br /> 4 licensing authority shall provide application forms for each license. <br /> 5 9.2 The applicant must complete and sign the application. If the <br /> 6 application is for a partnership, a partner must sign and the names of the <br /> 7 partners in full shall be given with their current residences. If the <br /> 8 application is for a corporation, an authorized officer thereof must sign <br /> 9 and the names of the officers of the corporation together with the location <br /> 10 of the principal office or place of business of such corporation must be <br /> 11 provided. <br /> 12 9.3 The licensing authority shall approve or deny all applications <br /> 13 for licenses required hereunder. <br /> 14 9.4 No license shall be issued pursuant to the provisions of this <br /> 15 chapter to the following persons: <br /> 16 <br /> (a) Any person who is under eighteen (18) years of age at the <br /> 17 time of application except as may specifically otherwise be <br /> provided in this chapter. <br /> 18 <br /> (b) Any person who has been convicted of a felony or <br /> 19 misdemeanor, excluding minor traffic violations, if the <br /> felony or misdemeanor for which he was convicted directly <br /> 20 relates to the license sought or any activity to be <br /> conducted hereunder, and <br /> 21 <br /> (i ) the time elapsed from the felony is less than ten <br /> 22 years from date of application; or <br /> 23 (ii ) the time elapsed since the misdemeanor is less than <br /> three years from date of application. <br /> 24 <br /> 25 This subsection shall not preclude the consideration of any prior <br /> 26 conviction of a felony or misdemeanor as a factor in determining whether or <br /> 27 not a license should be issued. <br /> 28 9.5 No license shall be issued unless the premises where the activity <br /> 99 is to be conducted complies with the requirements of all governmental <br /> 30 agencies, including, but not limited to, federal , state, city and county <br /> ;i1 laws or ordinances relating to buildings, fire, health, sanitation, zoning, <br /> i2 <br /> 5 <br />
