In the event the participant fails to contact the HopeWorks work crew manager by the deadline specified, <br /> HopeWorks W.C.M.shall notify the referring entity. <br /> Contact for purposes of work crew shall constitute appearing in person at specified date,time,and location. Failure <br /> to do so will result in termination of the referral and HopeWorks obligation under this agreement for such referral. <br /> The City and HopeWorks retain the discretion to accommodate failure to contact and to continue working with <br /> participant who shows willingness based upon agreement of all parties. <br /> Initital Contact: <br /> When a participant contacts the HopeWorks W.C.M.to participate in Safe Streets Work Crew,HopeWorks W.C.M. <br /> will obtain basic statistical information for intake and enrollment. <br /> Intake/Enrollment: <br /> During this intitial meeting,the work crew manager will explain the details of the participant's program including all <br /> of the program rules and the number of days required for completion of the program as designated by the referring <br /> entitiy. The W.C.M.will explain this program is a voluntary alternative program before the participant signs the <br /> agreement,(voluntary is described in the form as well as a memorandum of understanding with the detail of the <br /> programs expectations),and the participant will be offererd time to consult with an attorney if they choose. <br /> Participants that wish to consult counsel will be scheduled an appointment within one wek to either enter into the <br /> program or have the case returned to the prosecutor for charging decision. <br /> Program: <br /> WORK CREW: Work Crew is designed to last between 2-8 days with the exception to those wishing to utilize the <br /> program for community service hours,and shall be designatied by the referring entitiy. A day of work crew begins at <br /> 8:45 a.m.and runs until 3:00 p.m. Lunch shall be provided by HopeWorks as well as all materials necessary to <br /> perform assigned work. Participants can successfully complete the program if for the assigned days,they: <br /> *appear on time each day for work crew at he work crew office and/or a designated location by the W.C.M. <br /> *actively participate in clean-up activities as instructed by the work-crew supervisor. <br /> *active participation in small group exercises andor workbook activities <br /> *return all work crew tools,clothing,and equipment <br /> Statistics: <br /> On a monthly basis,the HopeWorks W.C.M.will provide the City with statistical reports outlining refereal, <br /> enrollments,completions and failtures. <br /> Page 13 <br /> (Form Approved by City Attorney's Office November 1,2009) <br />
