• • • 1 from any and all claims and risks in connection with any activity performed 2 by the licensee by virtue of this Chapter and provide the following minimum 3 coverage: 4 1 . $500,000 per person, per occurrence. 5 2. $500,000 annual aggregate. 6 7 Said policy must specifically name the City of Everett, its officers, 8 employees, and agents, as an additional insured party thereunder in the 9 following manner: 10 "The City of Everett, its officers, employees and agents are an 11 additional insured for all coverages provided by this policy of insurance and shall be fully and completely protected by this 12 policy and for any claim, suit, injury, death, damage or loss of 13 any sort sustained by a person, organization or corporation in connection with any activity upon or use or occupancy of establishments regulated by this Chapter. 14 The coverages provided by this policy to the City of Everett, its 15 officers, employees or agents or any other named insured shall not be terminated, reduced, or otherwise changed in any respect 16 without providing at least thirty (30) days prior written notice 17 to the City Clerk of the City of Everett." 18 C. The licensee shall deliver to the City Clerk a copy of all 19 policies required under this provision and all endorsements thereto or other 20 evidence to the reasonable satisfaction of the City Clerk that the licensee 21 has secured or renewed and is maintaining insurance as required by this 22 section. 23 The "ACORD" form of Certification of Insurance shall not be 24 submitted as such evidence, and shall not be deemed to be satisfactory 25 evidence unless the following changes are made on such form: 26 27 The wording on the top of the form: 28 "This certificate is issued as a matter of information only and confers no rights upon the certificate holder." 29 30 shall be deleted in its entirety. 31 32 11
