• <br /> 1 Section 5: LICENSE - APPLICATION - REQUIREMENTS. <br /> 2 The person desiring to conduct and/or operate a public dance shall be <br /> 3 responsible for obtaining a public dance hall license. Each applicant shall <br /> 4 complete an application which shall include the following information: <br /> 5 <br /> 6 A. 1 . The name and address (home and business) of the <br /> applicant; <br /> 7 2. The name and address of the operator of the public <br /> 8 dance hall ; <br /> 9 3. The name and address of the owner of the premises <br /> upon which the public dance hall is located; and <br /> 10 4. An oath or affirmation that the information listed is <br /> 11 true under penalty of perjury. <br /> 12 B. If the answers to Section 5(A) (1 )-(4) involve an unincorporated <br /> 13 association, corporation or partnership, the name of the unincorporated <br /> 14 association, corporation or partnership, and the names and addresses of the <br /> 15 limited partners, partners, officers and directors thereof shall be provided. <br /> 16 C. A written statement setting forth all measures used to insure <br /> 17 that adequate traffic control and crowd protection, both within and without <br /> 18 the premises, will be maintained. <br /> 19 D. Proof of indemnification and insurance as required by Section 15. <br /> 20 E. A statement from the applicant that the premises are in <br /> 21 compliance with all City laws including, but not limited to, building, <br /> 22 zoning, noise and fire codes. <br /> 23 F. Such other information as the City Clerk requires by rule adopted <br /> 24 pursuant to this Chapter for the health, safety and welfare of persons <br /> 25 attending public dances. <br /> 26 Section 6: LICENSE - DENIAL OF APPLICATION. <br /> 27 The City Clerk shall deny a license if: <br /> 28 A. The applicant has failed to comply with all State, County and <br /> 29 City laws which shall include, but are not limited to, building, zoning, <br /> 30 health, noise and fire codes. <br /> 31 <br /> 32 <br /> 4 <br />
