• <br /> 1 9. Where additional pretreatment and/or operation and <br /> maintenance activities will be required to comply with this <br /> 2 ordinance, the Discharger shall provide a declaration of <br /> the shortest schedule by which the Discharger will provide <br /> 3 such additional pretreatment and/or implementation of <br /> additional operational and maintenance activities. <br /> 4 <br /> a. The schedule shall contain milestone dates for the <br /> 5 commencement and completion of major events leading <br /> to the construction and operation of additional <br /> 6 pretreatment required for the Discharger to comply <br /> with the requirements of this ordinance including, <br /> 7 but not limited to, dates relating to hiring an <br /> engineer , hiring other appropriate personnel, <br /> 8 completing preliminary plans, completing final plans, <br /> executing contract for major components, commencing <br /> 9 construction, completing construction, and all other <br /> acts necessary to achieve compliance with this <br /> 10 ordinance. <br /> 11 b. Under no circumstances shall the Director permit a <br /> time increment for a single step directed toward <br /> 12 compliance which exceeds nine (9) months. <br /> 13 c. Not later than 14 days following each milestone date <br /> in the schedule, and the final date for compliance, <br /> 14 the Discharger shall submit a progress report to the <br /> Director , including no less than a statement as to <br /> 15 whether or not it complied with the increment of <br /> progress represented by that milestone date, and, if <br /> 16 not, the date on which it expects to comply with this <br /> increment of progress, the reason for delay, and the <br /> 17 steps being taken by the Discharger to return the <br /> construction to the approved schedule. In no event <br /> 18 shall more than 9 months elapse between such progress <br /> reports to the Director . <br /> 19 <br /> 10. Disclosure of each product produced by type, amount, <br /> 20 process or processes, and rate of production. <br /> 21 11. Disclosure of the type and amount of raw materials <br /> (including chemicals) utilized (average and maximum per <br /> 22 day) . A Materials Safety Data Sheet shall also be required. <br /> 23 12. The signature of an authorized official of the applicant. <br /> 24 <br /> The Director will evaluate the complete application and data furnished <br /> 25 <br /> by the Discharger and may require additional information. Within 30 days <br /> 26 <br /> after full evaluation and acceptance of the data furnished, the Director <br /> 27 <br /> shall issue a Wastewater Discharge Permit subject to terms and conditions <br /> 28 <br /> provided herein. <br /> 29 <br /> All sewers shall have an inspection and sampling manhole or structure <br /> 30 <br /> with an opening of no less than 24 inches diameter and an internal diameter <br /> 31 <br /> of no less than 48 inches. The Director may require inspection and sampling <br /> 32 <br /> 12 <br />
