• <br /> • <br /> 1 Wastewater Discharge Permit within 120 days of being notified by the City. <br /> 2 New source Dischargers shall obtain a Wastewater Discharge Permit prior to <br /> 3 discharging. <br /> 4 C. Permit Application. Significant Industrial Dischargers shall <br /> 5 complete and file with the Director a permit application in the form <br /> 6 prescribed by the Director . The permit application filed with the Director <br /> 7 shall be accompanied by the appropriate fee. Existing Significant <br /> 8 Industrial Dischargers shall apply for a Wastewater Discharge Permit within <br /> 9 60 days of being notified by the City, and proposed new Dischargers shall <br /> 10 apply at least 90 days prior to connecting to the Municipal Sewer System. <br /> 11 No discharge permit shall be issued unless and until the following <br /> 12 information has been provided: <br /> 13 <br /> 1. Disclosure of name, address, and location of the Discharger . <br /> 14 <br /> 15 2. Disclosure of Standard Industrial Classification (SIC) <br /> number according to the Standard Industrial Classification <br /> 16 Manual, Bureau of the Budget, 1972, as amended. <br /> 3. Disclosure of wastewater constituents and characteristics <br /> 17 <br /> as requested by the Director and determined by bona fide <br /> 1g chemical and biological analyses. Sampling and analysis <br /> shall be performed in accordance with procedures <br /> 19 established by the EPA and contained in 40 CRF, Part 136, <br /> as amended. <br /> 20 4. Disclosure of time and duration of discharges. <br /> 21 5. Disclosure of average daily and instantaneous peak <br /> 22 wastewater flow rates, in gallons per day, including daily, <br /> monthly and seasonal variations, if any. All flows shall <br /> be measured unless other verifiable techniques are approved <br /> 23 by the Director due to cost or nonfeasibility. <br /> 24 6 . Disclosure of site plans, floor plans, mechanical and <br /> 25 plumbing plans and details to show all sewers, sewer <br /> connections, inspection manholes, sampling chambers and <br /> 26 appurtenances by size, location, and elevation. <br /> 27 7 . Description of activities, facilities, and plant processes <br /> on the premises including all materials which are or may be <br /> 28 discharged to the Municipal Sewer System. <br /> 29 8. Disclosure of the nature and concentration of any <br /> pollutants or materials prohibited by this ordinance in the <br /> 30 discharge, together with a statement regarding whether or <br /> not compliance is being achieved with this ordinance on a <br /> 31 consistent basis and if not, whether additional operation <br /> and maintenance activities and/or additional pretreatment <br /> 32 is required for the Discharger to comply with this <br /> ordinance. <br /> 11 <br />
