The City Clerk shall issue the license promptly upon <br /> receipt of the application, the information required <br /> 4 to be in the application, the investigation by the <br /> Police Department in accordance with Section 1(E) (4) , <br /> 5 and the license fee. <br /> 6 3. If any person or entity acquires, subsequent to the <br /> issuance of a public place of adult entertainment license <br /> 7 for places offering adult entertainment, a substantial <br /> interest, as defined in Section 1(E) (1) (c) , in the licensed <br /> 8 premises, notice of such acquisition shall be provided in <br /> writing to the City Clerk forthwith. The information <br /> 9 required to be provided pursuant to this section shall be <br /> that information required pursuant to Section 1(E) (1) (c) of <br /> 10 this chapter. <br /> 11 4. The Clerk, upon presentation of such applications and <br /> before acting upon the same, shall refer such applications <br /> 12 to the Police Department, which shall make a full <br /> investigation as to the truth of the statements contained <br /> 13 therein. <br /> 14 <br /> be and the same is hereby amended to read as follows: <br /> 15 <br /> 16 E. License Applications. <br /> 17 1. Public Place of Adult Entertainment License. <br /> 18 All applications for a public place of adult entertainment <br /> license shall be submitted in the name of the person or <br /> 19 entity proposing to conduct such activity on the business <br /> 20 premises and shall be signed by such person or his agent <br /> and notarized or certified as true under penalty of <br /> 21 <br /> perjury. All applications shall be submitted on a form <br /> supplied by the City Clerk, which shall require the <br /> following information: <br /> 22 <br /> a. The name, home address, home telephone number, date <br /> 23 and place of birth, and social security number of the <br /> 24 applicant if the applicant is an individual. <br /> 25 b. The business name, address and telephone number of <br /> the establishment. <br /> 26 c. The names, addresses, telephone numbers, and social <br /> 27 security numbers of partners, corporate officers or <br /> shareholders who collectively own fifty-one percent <br /> 28 (51%) or more of the business. <br /> 29 d. The name, address and telephone number of the owner <br /> of the property on which the public place of adult <br /> 30 entertainment is located. <br /> 31 2. Application for Manager or Entertainer License. <br /> 32 All applications for a manager's or entertainer's license <br /> shall be signed by the applicant and notarized or certified <br /> 3 <br />
