• <br /> 1 to be true under penalty of perjury. At the time of <br /> application, a picture shall be taken of the applicant by <br /> 2 the City Clerk. All applications shall be submitted on a <br /> form supplied by the City Clerk, which shall require the <br /> 3 following information: <br /> 4 a. The applicant's name, home address, home telephone <br /> number, date and place of birth, social security <br /> 5 number, and any stage names or nicknames used in <br /> entertaining. <br /> 6 <br /> b. With the application the applicant shall present <br /> 7 picture identification which shall include (i) a <br /> motor vehicle operator's license issued by any state <br /> 8 bearing the applicant's photograph and date of birth; <br /> or (ii) an identification card bearing the <br /> 9 applicant's photograph and date of birth. <br /> 10 c. The City Clerk shall issue the license promptly upon <br /> receipt of the application, the information required <br /> 11 to be in the application, the investigation by the <br /> Police Department in accordance with Section 1(E) (4) , <br /> 12 and the license fee. <br /> 13 3. If the identity of any person or entity defined in Section <br /> 1(E) (1) (c) changes, notice shall be provided in writing to <br /> 14 the City Clerk forthwith. The information required to be <br /> provided pursuant to this section shall be that information <br /> 15 required pursuant to Section 1(E) (1) (c) of this chapter. <br /> 16 4. The Clerk, upon presentation of such applications and <br /> before acting upon the same, shall refer such applications <br /> 17 to the Police Department, which shall make a full <br /> investigation as to the truth of the statements contained <br /> 18 therein. <br /> 19 <br /> Section 4: That Section 1(K) (2) of Ordinance No. 1196--85 which reads <br /> 20 <br /> as follows: <br /> 21 <br /> 22 The license application shall be approved or disapproved within <br /> ten (10) working days from the date of filing a completed <br /> 23 application which complies with the requirements of this chapter, <br /> unless the applicant agrees to an extension of the time period in <br /> 24 writing. <br /> 25 In the event that the City denies a license, the City shall <br /> within the aforesaid ten (10) day period notify the applicant of <br /> 26 the denial and the reasons therefor. The denial shall be for a <br /> period of time not to exceed one year. Should the applicant <br /> 27 disagree with the City's determination, he must file a notice of <br /> nonacceptance with the City Attorney's Office within (2) working <br /> 28 days of receipt of the notification of denial. <br /> 29 The City shall, within five (5) working days following receipt of <br /> the notice of nonacceptance, apply to the Snohomish County court <br /> 30 for a judicial determination as to whether the applicant's <br /> license was properly denied. The burden of showing that the <br /> 31 applicant's license was properly denied shall rest on the City. <br /> 32 <br /> 4 <br />
