Project: Hayes Street Regulator& CSO Control <br /> Change Order No.1 <br /> Change Order Effective Date: 10/18/17 <br /> JUSTIFICATION: <br /> Item#5—Traffic Control Labor. The deleted hours for Traffic Control Labor will not be needed to <br /> complete Work on the project. <br /> Item#12—Crushed Surfacing Base Course,and Item#13—HMA CL 'A-inch PG 64-22. <br /> Additional area requiring grading and paving along Hayes Street is being added to the project, <br /> resulting in additional tons of Crushed Surfacing Base Course and tons of HMA CL '/s inch PG 64-22 <br /> to be incorporated into the project. <br /> Deleting Item#21—LS#9 Structure,for Screening Equipment, and adding new Item#54—LS <br /> #9 Modified Screen Vault Structure. <br /> The original design called for the Contractor to provide a new lid on existing manhole W02, adjacent <br /> to Lift Station#9, to accept new screening equipment and provide a new 6' by 6' by 4' deep precast <br /> vault adjacent to existing manhole W02, as the debris receptacle. After reviewing the geometry of the <br /> pipe flows entering into existing manhole W02 and coordinating with the mechanical screening <br /> equipment supplier,the Engineers determined that the new mechanical screening equipment would <br /> work more effectively if a new modified screen vault structure could be built in the location of existing <br /> manhole W02, which would incorporate both the new screening equipment and the removable debris <br /> receptacle. Furthermore,the new larger vault would make it possible to install slightly larger screen <br /> equipment thus doubling the flow capacity of the screen unit. The new modified Screen Vault <br /> Structural design is approximately 14' by 8' by 21' deep and requires the cast-in-place approach, as it <br /> has to match up adjacent to an existing concrete block off the edge of lift station#9.This new cast-in- <br /> place structure is significantly larger in plan view and deeper in depth,than the previously designed 6' <br /> by 6' by 4' deep,precast vault, originally called for in the bid documents. <br /> Item#32--Control Density Fill(CDF) for Filling Pipes. The deleted cubic yards of Control Density <br /> Fill (CDF)for Filling Pipes,will not be needed to complete Work on the project. <br /> CONTRACT TIME: <br /> The"Time for Completion", section#2 on page one of the Contract,required Work to be Substantially <br /> Complete within 125 working days of the effective date of Notice to Proceed,and Physically complete <br /> within 244 calendar days of the effective date of Notice to Proceed or by December 31,2017, <br /> whichever occurs first. <br /> 1 <br /> Special Construction Constraints section 1-08.4(2), as modified by Addendum#1, allows for the <br /> installation of mechanical screen equipment @ Lift Station#9, site restoration,paving and <br /> demobilization to occur after December 31,2017. <br /> The City of Everett modified the design of the screen vault structure and approved the manufactures <br /> screening equipment shop drawings on 10/19/17. This screening equipment has a 14 to 16 week lead <br /> time for manufacturing. Therefore the new screening equipment should be ready for installation by <br /> late January, or early February 2018. Forty Five(45) calendar days will be added to the contract time <br /> by this change order#1, which makes the new date for Physical Completion,February 14,2018. <br /> Page 4 of 4 <br />
