City of Everett Human Needs Grant • <br /> January 1 through December 31, 2018 <br /> Catholic Community Services <br /> Volunteer Chore Services and Family and Children Services <br /> EXHIBIT A: Program Summary <br /> PROGRAM OBJECTIVE <br /> The recruiter secures volunteers who serve elders and adults who have functional disabilities, to <br /> provide the support they need to live with dignity in their own homes and to provide access to health <br /> and social services that will help them remain an active part of our community. <br /> PROGRAM COMPONENTS AND STRUCTURE: <br /> Volunteer Chore Services has 8 components: <br /> 1) Client Intake <br /> 2) Client in-home assessment <br /> 3) Volunteer Recruitment <br /> 4) Volunteer Orientation <br /> 5) Volunteer and client match <br /> 6) Volunteer training <br /> 7) Client follow-up <br /> 8) Volunteer follow-up concerning match between client and volunteer. <br /> Client referrals come from various sources including D.S.H.S, doctor's offices, hospital social <br /> workers, insurance providers, relatives, and neighbors, and self-referrals. Volunteers provide <br /> services for-clients and assist with activities of daily living.Their interaction with the clients minimizes <br /> safety concerns and the isolation which is frequently experienced by elders and bridges the service <br /> gap for persons ineligible for paid chore services. <br /> Who is to be served: <br /> Low-income functionally disabled adults and elders are the target population. These are the adults <br /> and elders who may"fall through the cracks"due to lack of family support and financial resources. <br /> How and when will the service be provided: <br /> Volunteer chore services are provided in the client's homes. The schedule is arranged to meet both <br /> the client's need and the volunteer's availability. Services are provided throughout the week as well <br /> as in the evenings or on the weekend. Catholic Community Services office is open Monday thru <br /> Friday from 8:30 AM to 5 PM; however, staff is available evening and weekends for pre-arranged <br /> appointments. <br /> Service Providers: <br /> The services are provided by trained volunteers. Volunteers come from a variety of different <br /> backgrounds and skill set. Each volunteer is required to complete a background check, provide <br /> personal references and attend an orientation process. Volunteers provide our community with <br /> hours of needed services in health, education,social services, and in the arts and recreation. <br />
