City of Everett Human Needs Grant <br /> January 1 through December 31, 2018 <br /> Cocoon House <br /> Cocoon House Central: Emergency Teen Shelter <br /> EXHIBIT A: Program Summary <br /> Cocoon House Central Shelter is an eight (8) bed residential facility providing temporary <br /> housing and supportive services to homeless and at-risk youth. The program is licensed <br /> to serve youth ages 12 through 17. Cocoon House staff work one-on-one with each <br /> youth to create and implement an individualized care plan focused on safe housing <br /> (through family reunification whenever possible), strengthening a youth's connection to <br /> school, and building skills for self-sufficiency. In PY 2018, Cocoon House Central will <br /> serve 120 youth, at least 50 of those being Everett residents. Specific program outputs <br /> and outcomes include: <br /> • At least fifty (50) unduplicated youth from Everett receive housing and supportive <br /> services <br /> • At least 500 bed nights are provided (with 62 of those funded by City of Everett <br /> Human Needs) <br /> • At least seventy-five percent (75%) of youth will exit directly to safe housing with <br /> a priority on family reunification <br /> • At least seventy-five percent (75%) of youth have a positive school status at the <br /> time of exit <br /> • At least eighty-five percent (85%) of youth will demonstrate independent living <br /> skills while in the program <br />
