1 d. The name and address of the owner of the business 2 where the panoram devices are located as described in subsection a. 3 3. All applications for a manager's license shall be signed by the 4 applicant and notarized or certified to be true under penalty of perjury. At the time of application a picture shall be taken of the 5 applicant by the City Clerk. All applications shall be submitted on a form supplied by the City Clerk, which shall require the following 6 information: 7 a. The applicant's name, home address, home telephone number, date and place of birth, and social security 8 number. 9 b. With the application the applicant shall present picture identification which shall include (i) a motor vehicle 10 operator's license issued by any state bearing the applicant's photograph and date of birth; or (ii) an identification card 11 bearing the applicant's photograph and date of birth. 12 c. The City Clerk shall issue the license promptly upon receipt of the application, the information required to be in the 13 application, the investigation by the Police Department in accordance with Section 1(G)(5), and the license fee 14 4. If any person or entity acquires, subsequent to the issuance 15 of a panoram premise license, a substantial interest, as defined in Section 1(G)(1)(c), in the licensed premises, notice 16 of such acquisition shall be provided in writing to the City Clerk forthwith. The information required to be provided 17 pursuant to this section shall be that information required pursuant to Section 1(G)(1)(c) of this chapter. 18 5. The Clerk, upon presentation of such panoram premise, panoram 19 device and manager's applications and before acting upon the same, shall refer such applications to the Police Department, which shall 20 make a full investigation as to the truth of the statements contained therein. 21 22 H. Licensing - Compliance With Other City Ordinances. 23 All other City approvals and permit issuance other than those specifically set forth herein including but not limited to fire, 24 building and zoning, are separate from the licensing process set forth in this chapter, notwithstanding Chapter 5.04 E.M.C. The 25 granting of any license or the providing of any approval pursuant to this chapter shall not be deemed to be an approval of any City 26 permit or approval not specifically set forth in this chapter. 27 I. Inspections. 28 Licensees operating premises and devices licensed under this 29 chapter shall hold those areas upon the premises which are accessible to the public and the devices therein open for routine regulatory inspections 30 by the City during business hours to ensure compliance with the require- ments of this chapter. This section shall not restrict or limit the right of 31 entry vested in any law enforcement agency or the Fire Department. 32 -4-
