1 d. The name and address of the owner of the business <br /> 2 where the panoram devices are located as described <br /> in subsection a. <br /> 3 3. All applications for a manager's license shall be signed by the <br /> 4 applicant and notarized or certified to be true under penalty of <br /> perjury. At the time of application a picture shall be taken of the <br /> 5 applicant by the City Clerk. All applications shall be submitted on <br /> a form supplied by the City Clerk, which shall require the following <br /> 6 information: <br /> 7 a. The applicant's name, home address, home telephone <br /> number, date and place of birth, and social security <br /> 8 number. <br /> 9 b. With the application the applicant shall present picture <br /> identification which shall include (i) a motor vehicle <br /> 10 operator's license issued by any state bearing the applicant's <br /> photograph and date of birth; or (ii) an identification card <br /> 11 bearing the applicant's photograph and date of birth. <br /> 12 c. The City Clerk shall issue the license promptly upon receipt <br /> of the application, the information required to be in the <br /> 13 application, the investigation by the Police Department in <br /> accordance with Section 1(G)(5), and the license fee <br /> 14 4. If any person or entity acquires, subsequent to the issuance <br /> 15 of a panoram premise license, a substantial interest, as <br /> defined in Section 1(G)(1)(c), in the licensed premises, notice <br /> 16 of such acquisition shall be provided in writing to the City <br /> Clerk forthwith. The information required to be provided <br /> 17 pursuant to this section shall be that information required <br /> pursuant to Section 1(G)(1)(c) of this chapter. <br /> 18 5. The Clerk, upon presentation of such panoram premise, panoram <br /> 19 device and manager's applications and before acting upon the same, <br /> shall refer such applications to the Police Department, which shall <br /> 20 make a full investigation as to the truth of the statements <br /> contained therein. <br /> 21 <br /> 22 H. Licensing - Compliance With Other City Ordinances. <br /> 23 All other City approvals and permit issuance other than <br /> those specifically set forth herein including but not limited to fire, <br /> 24 building and zoning, are separate from the licensing process set <br /> forth in this chapter, notwithstanding Chapter 5.04 E.M.C. The <br /> 25 granting of any license or the providing of any approval pursuant to <br /> this chapter shall not be deemed to be an approval of any City <br /> 26 permit or approval not specifically set forth in this chapter. <br /> 27 I. Inspections. <br /> 28 Licensees operating premises and devices licensed under this <br /> 29 chapter shall hold those areas upon the premises which are accessible to <br /> the public and the devices therein open for routine regulatory inspections <br /> 30 by the City during business hours to ensure compliance with the require- <br /> ments of this chapter. This section shall not restrict or limit the right of <br /> 31 entry vested in any law enforcement agency or the Fire Department. <br /> 32 <br /> -4- <br />
