Project: WFP East Clearwell Roof Replacement
Change Order No.01
Change Order Effective Date: 3/15/18
CONTRACTOR
By Date: '415) i
0,y'i Ask.,Officer
12ft1G1 1
Exhibit A - Description of Changed Work
Work Change Directive —No. 001 Investigate Possible Void Under Floor Slab:
WCD-001 is a lump sum amount revision of$4,480.00. WCD-001 did not add any
calendar days.
Justification: Prior to the start of construction, the City's consultant performed an
examination of the East Clearwell using ground penetrating radar. The ground
penetrating radar identified a possible void under the floor slab in an area where past
settlement of the clearwell wall had occurred. The City had our design-build contractor
core a hole through the slab in the area of the possible void. The foundation material
under the slab was probed to check for a void and to confirm the material was
consolidated.
WCD-001 covers the additional costs to core the hole and repair the hole using
reinforcing steel and placing new concrete in hole.
Work Change Directive — No. 002: Install Handrail on the North Bridge
Abutment: WCD-002 is a lump sum revision for $3,750.00. WCD-002 did not add any
calendar days.
Justification: The project scope included the removal and replacement of the bridge
between the south dam and the Screen House intake structure. The old bridge and
abutment were 20-feet wide. The new bridge is 6-feet wide. This left 14-feet of fall
exposure along the existing abutment. The added handrail will protect personnel from
possible falls.
WCD-002 covers the additional costs to fabricate, galvanize and install the handrail.
Work Change Directive — No. 003 — Revise Floor Repair Area: WCD —No. 003 is a
lump sum revision for $59,468.07 and the addition of 10-calandar days.
WFP East Clearwell Roof Replacement March 15, 2018
WO#UP3662 006363 - 3
