Project: WFP East Clearwell Roof Replacement <br /> Change Order No.01 <br /> Change Order Effective Date: 3/15/18 <br /> Justification: The project scope included repairs to the surface of the concrete floor <br /> slab in the East Clearwell. The design anticipated that approximately 25% of the floor <br /> slab would require resurfacing. After our design-build contractor began work repairing <br /> the floor surface, it was discovered the delamination of the concrete floor surface was <br /> spread throughout the entire floor slab. <br /> WCD-003 covers the additional costs to increase the repair area from 25% to 100% of <br /> the floor slab. <br /> Work Change Directive— No. 004 Repair Concrete Spalls on North Wall of the <br /> East Clearwell: WCD —No. 004 is a lump sum revision of$6,577.06. WCD-004 did <br /> not add any calendar days. <br /> Justification: After the existing roof was removed, two concrete spalls were found on <br /> the north wall of the east clearwell. The spalls occurred at an expansion joint and were <br /> caused by past settlement of two wall sections. Due to the past settlement, the top <br /> sections of the walls pushed against each other causing the concrete to crack and spall. <br /> The spalled area was covered by the roof and was not visible until the roof was <br /> removed. The depth of the spalls was approximately half the thickness of the wall and <br /> required repairs. <br /> WCD-004 covers the additional costs to saw-cut, remove the damaged section, dowel <br /> and epoxy new reinforcing steel and replace the damaged concrete with new concrete. <br /> Work Change Directive— No. 005 — Replace the Existing Suction Pipe for the <br /> Recovered Water Sample Pump: WCD-005 is a lump sum revision of$1,526.60. <br /> WCD-005 did not add any calendar days. <br /> Justification: The existing sample pump suction pipe had obstructions inside the pipe. <br /> WFP personnel planned to replace the pipe. Our design-build contractor was working in <br /> the same area where the suction pipe was buried. Our contractor was replacing a section <br /> of the recovered water pipe where the sample pump's suction pipe tied into. In order to <br /> replace the recovered water pipe, our design-build contractor had a section of the sample <br /> pump's suction pipe exposed. The work our design-build contractor was doing was time <br /> sensitive. By having our design-build contractor replace the sample pump's obstructed <br /> pipe, we avoided having multiple crews working in the same small area. Our design- <br /> build contractor was able to replace the sample pump pipe while they also worked on <br /> replacing a section of the recovered water pipe, thus saving time. <br /> WFP East Clearwell Roof Replacement March 15, 2018 <br /> WO#UP3662 006363 -4 <br />
