Project: WFP Operations Building Seismic Retrofit -Rebid
Change Order No.003
Change Order Effective Date: 1/25/18
Revised 2/14/18
Date: 1-/(/r7// Date:
Date: 2h1bY Date: Z`(it(iS
CONTRACTOR
By ' __ __._ Date:
Z-//c///
Officer
Exhibit A - Description of Changed Work
Work Change Directive—No. 007 Revisions Due to Soffit Elevation Difference:
WCD-007 is a lump sum amount revision of$9,186.00 and the addition of 2-working
days.
Justification: The structural design required a continuous angle along the west side of
the Operations Building. This angle was installed between west wall and the ceiling of
the first floor. The elevation of the ceiling changed near the northwest corner of the
building. Because of the elevation difference, the per plan angle would not be
continuous. A new larger angle had to be fabricated and installed in the areas where the
ceiling was higher. The elevation difference was not obvious and was not shown in the
contract plans or the record drawings. This work change directive covers the additional
costs to fabricate a new larger angle, grout in the anchor bolt holes for the original, per
plan angle and drill new anchor bolt holes for the larger angle.
Work Change Directive— No. 008— Remove and Replace Existing Mechanical,
Electrical and Plumbing (MEP) Conflicting with Seismic Strengthening: WCD-008
is a lump sum revision for$11,556.42 and the addition of 3-working days.
Justification: Contract plans do not identify all MEP items that conflict with or prevent
the installation of seismic strengthening. Existing MEP items were to be removed by
City personnel.
City personnel removed the items called out in the plans. After completing the
removals, personnel were assigned to other time sensitive projects and did not have the
time to remove items that were not identified in the plans. As a result, the Contractor
was directed to remove, or remove and reinstall MEP items.
WFP Operations Building Seismic Retrofit-Rebid January 25,2018
WO#UP3600 006363 -4
