Project: Three Lakes Valve Bypass <br /> Change Order No.1 <br /> Change Order Effective Date: March 20, 2018 <br /> vault, and to account for costs to construct a concrete lid. The concrete slab lid will provide a <br /> safer and more secure cover. <br /> 3. Reinforced Concrete Pad for Generator. The structural engineer determined that placing the <br /> backup generator on top of the vault slab would add unnecessary weight to the structure. This <br /> new item is being added to account for materials costs to construct concrete generator pad in <br /> location adjacent to vault. <br /> 4. Add Air/Vacuum Valve Schedule 80 piping, restrained to vault with anchor bolts. This <br /> new item is being added to vent the Air/Vacuum valve to the outside of the vault, rather than <br /> discharging into the vault, and to account for costs for the added work. <br /> 5. Eliminate Concrete Encasement for 24-Inch Access Manway, Detail Drawing D4: The <br /> approved drawings called for a reinforced concrete encasement for each 24-inch access <br /> manway, which included concrete and rebar. The City of Everett maintenance and operations <br /> groups said the concrete was not needed or desired and thus were eliminated, resulting in a <br /> credit to the City of Everett. <br /> New Item# 102 —Additional Restrained Welds. The plans called for welding three exterior pipe <br /> joints for the existing 51-Inch Water Transmission Line upstream and downstream of the new 51"x36" <br /> wye segments. The welding of the pipe joints is to provide enough thrust control so as to avoid a joint <br /> failure when the bypass piping is put into use. While excavating to prepare for the wye connections, <br /> three additional pipe joints were found that needed to be welded to provide the necessary thrust <br /> control. This item is being added to account for costs to weld three additional pipe joints. <br /> New Item # 103—Additional Labor and Equipment Costs. Because this project is a Lump Sum <br /> project, it did not include a force account bid item to pay for additional costs for miscellaneous <br /> changes to the Contract. This item is being added to account for additional labor and equipment costs <br /> due to added work that was not part of the original Contract as described below. <br /> 1. Excavation costs for additional joint welding. <br /> 2. Place pipe bedding, backfill, and compact trench excavation for additional joint welding. <br /> 3. Move jobsite trailer and storage Conex for access to pipe for additional joint welding. <br /> 4. Core 8-Inch opening into concrete vault for 8-inch Air/Vacuum Valve. <br /> 5. Construct reinforced concrete pad for backup power generator. <br /> New Item # 104—Early Finish Incentive Bonus. The Contract included an early finish incentive <br /> bonus of$7,500.00 per hour, up to a maximum of two hours (Contract Section 00 60 13-1.2D)to <br /> encourage the Contractor to expedite work during the water shutdown in order to ensure water <br /> customers had water as soon as possible. The Contractor was allowed 36 hours to complete the <br /> shutdown work and completed it within 24 hours, thus earning the two hour maximum bonus. This <br /> item is being added to account for the $15,000.00 bonus the Contractor earned. <br /> Contract Time: There are no calendar days added to this Contract. <br /> Page 4 of 4 <br />
