ARTICLE 12 - MEDICAL BENEFITS/INSURANCE <br /> 1. EMPLOYEE AND DEPENDENT MEDICAL: <br /> A. The City agrees to offer the City's self-insured and Kaiser Permanente plan. As <br /> an option, the City also agrees to allow employees and their dependents to <br /> choose the LEOFF Health and Welfare Trust plan A. The employee will make a <br /> 5% contribution to the cost of medical coverage. <br /> If the LEOFF Health and Welfare Trust plan A is elected in 2018, the employee <br /> shall pay the premium difference between the City's self-insured plan and the <br /> LEOFF Health and Welfare Trust plan A in addition to 5% of the City's self- <br /> insured plan premium. <br /> Effective January 1, 2019, in lieu of LEOFF Health and Welfare Trust Plan A, the <br /> City shall offer the LEOFF Health and Welfare Trust Plan B. The City shall pay <br /> ninety percent (90%) of the monthly Plan B premium for the employee and Trust <br /> eligible dependent(s) medical coverage through the LEOFF Health & Welfare <br /> Trust. The City will make contributions to an HRA in the amount of <br /> $166.66/month for employee only coverage or $333.33/month for family <br /> coverage if the employee enrolls in the LEOFF Trust Plan B. Contributions to the <br /> HRA will be made by the City and are subject to the rules and limitations <br /> contained within the Internal Revenue Code. The Medical plan will be <br /> administered by the LEOFF Health and Welfare Trust and their designee <br /> (Premera Blue Cross). The City will select and retain a third party to administer <br /> the HRA account(s). <br /> B. The employee has the choice of plans, but the plan must be the same for all <br /> family members. <br /> 2. DENTAL COVERAGE: The City will pay 100% of the monthly premium for the basic <br /> dental insurance plan offered through Washington Dental Service. Said dental <br /> insurance will be provided for all bargaining unit members and their dependents, with an <br /> annual maximum of$2,000. <br /> 3. In lieu of the City's historical practice of making a modest contribution toward the <br /> purchase of a disability income insurance plan for employees, the City will make a <br /> monthly contribution to the Premium Reimbursement Plan of the Washington State <br /> Council of Fire Fighters Employee Benefit Trust (the "Plan") on behalf of each employee. <br /> The monthly contribution will be the equivalent of one hour's pay for a First Class Fire <br /> Fighter at the then-applicable rate. <br /> By making such contributions, the City does not make any representations concerning <br /> the tax treatment of City contributions or of payments made by the Plan and relies on the <br /> Plan's representation that the Plan will properly withhold and report all such payments in <br /> accordance with applicable law, including for payments made to domestic partners. <br /> Further, the City does not agree to make any further contributions to the Plan for any <br /> other liability or claim beyond the monthly contributions described above. In the event of <br /> any claim by the Plan or any other entity or individual against the City, other than <br /> concerning the above-described monthly contributions, the Union agrees to indemnify <br /> 16 <br />
