Project: Portal 1 Replacement Change Order No.01 Change Order Effective Date: 6/21/18 CONTRACTOR Date: , , /� . fic Exhibit A - Description of Changed Work Work Change Directive —No. 01 Fabricate and Install Pipe Adapter: WCD-001 is a lump sum revision for $4,862.93. WCD-01 added two calendar days. Justification: After demolition of the existing portal structure, it was discovered that the existing Diversion Dam pipe had a slight taper. The prefabricated wall spool for the new portal structure would not fit the existing pipe due to the slight taper. WCD-02 covers the additional costs to fabricate and install an adapter between the existing pipe and the portal wall spool. Work Change Directive —No. 02 Repair Holes in the Existing 90" x 72" Reducer Pipe : WCD-02 is a lump sum revision for $2,748.64. WCD-02 added no calendar days. Justification: During the 1984 installation of PUD's 72" pipe, holes were drilled in the mortar lining to hang lights. In the 90" x 72" reducer, the holes were drilled too deep and penetrated the steel pipe. Prior to the Portal 1 Project, in 1984 the holes were found on startup and repaired. The repairs were found to be leaking after water was placed back into the pipe, prior to backfilling. Contract Time: Two Calendar Days are being added to the contract by this change order. They are for WCD-001, for the following reasons. A. Installation of the pipe adaptor required removal of the mortar lining and coating from the existing reducer pipe.
