Exhibit A-7 <br /> City of Everett <br /> Amendment No. 7 <br /> Cleveland Avenue Sewer System Replacement <br /> and Storm Water Separation <br /> WO# UP3398-34 <br /> Scope of Services <br /> Otak Project 31520.E <br /> Proposed services are provided as an amendment to professional service agreement, dated September <br /> 16, 2009 between Otak, Inc. (Consultant) and City of Everett (Client). <br /> Scope of Services <br /> The scope of services herein is for providing assist during project construction, more specifically <br /> described in detail as follows: <br /> 1. Assistance During Project Construction <br /> Task 500 - Construction Support Services <br /> The consultant will provide assistance-services during construction of the project. The City shall be <br /> responsible for leading the construction administration and construction observation effort. The <br /> Consultant will assist with public bid solicitation; engineering office support to review/respond to RFI and <br /> submittals; attending site/field meetings, when requested; and prepare project closeout documentation <br /> required by Department of Ecology. <br /> 500-501 Bid Assistance <br /> The Consultant will assist the City in soliciting, responding to questions, and evaluating bids for <br /> construction. The Consultant will assist in coordinating the electronic documents for online plan <br /> distribution; responding to contractor questions and document interpretation; attending one pre-bid <br /> meeting; preparing any required bid addenda; review bids to identify irregularities and/or verify contractor <br /> references, if requested; and post-bid preparation of"Conformed" Documents (editing to incorporate bid <br /> addenda). <br /> 500-502 Construction Administrative Tasks <br /> The Consultant will review construction submittals for document compliance, respond to contractor RFIs, <br /> and assist City staff with clarifications. The City shall be responsible for receiving all submittals, RFI and <br /> send to Consultant for review/response. <br /> Following construction, Consultant will prepare/assemble documentation on 1) operation and <br /> maintenance of the water quality treatment facilities and 2) Construction Quality Assurance Plan, as <br /> required by the Department of Ecology grant program. <br /> City of Everett 1 <br /> Cleveland Avenue Sewer System Replacement and Storm Water Separation Otak <br />
