CITY I-151-nage ',cnror Services Well-being as we age MEMORANDUM OF UNDERSTANDING between HOMAGE SENIOR SERVICES And CARL GIBSON SENIOR CENTER The purpose of this agreement is to set forth the essential elements of a cooperative working relationship between the parties to enhance the provision of SHIBA(Statewide Health Insurance Benefits Advisors)to assist clients that have Medicare and/or Healthcare concerns. SHIBA volunteers will assist the public with unbiased health insurance and access questions free of charge at: CARL GIBSON SENIOR CENTER,3025 Lombard Ave,Everett,WA 98201,USA SHIBA is a free, confidential and impartial counseling resource sponsored by the Washington State Office of the Insurance Commissioner.Volunteer advisors help clients understand their rights and options, and offer up-to-date information so that the client can make an informed decision. SHIBA volunteers are highly trained to counsel residents of all ages regarding their choices, options and problems with Medicare, private health insurance,affordable prescription drugs, and government programs. SHIBA volunteers and staff are also trained to counsel clients about Medigap coverage,employment-related health benefits, managed care, health care fraud/abuse, and health care resources for rural, low income and ethnic populations This agreement will be effective through January 2020 and may be extended,revised,or amended by mutual agreement with 30 days notice. To achieve the above, Homage Senior Services SHIBA Program will provide the following services: • SHIBA will staff CARL GIBSON SENIOR CENTER with a volunteer(s) who will provide up-to-date information so that the client can make an informed health care decision. • All SHIBA services are free of charge for the clients. • SHIBA will assist in marketing efforts for the services. • SHIBA hours will be held the second and fouth Thursday of the month, or at a time mutually agreed to by the CARL GIBSON SENIOR CENTER and the SHIBA volunteer. • Open Enrollment events will be scheduled at mutually agreed upon times. The CARL GIBSON SENIOR CENTER agrees to: • The CARL GIBSON SENIOR CENTER will provide adequate facilities to welcome and seat clients.
