CITY <br /> I-151-nage <br /> ',cnror Services <br /> Well-being as we age <br /> MEMORANDUM OF UNDERSTANDING <br /> between <br /> HOMAGE SENIOR SERVICES <br /> And <br /> CARL GIBSON SENIOR CENTER <br /> The purpose of this agreement is to set forth the essential elements of a cooperative working <br /> relationship between the parties to enhance the provision of SHIBA(Statewide Health <br /> Insurance Benefits Advisors)to assist clients that have Medicare and/or Healthcare concerns. <br /> SHIBA volunteers will assist the public with unbiased health insurance and access <br /> questions free of charge at: CARL GIBSON SENIOR CENTER,3025 Lombard Ave,Everett,WA <br /> 98201,USA <br /> SHIBA is a free, confidential and impartial counseling resource sponsored by the Washington <br /> State Office of the Insurance Commissioner.Volunteer advisors help clients understand their <br /> rights and options, and offer up-to-date information so that the client can make an informed <br /> decision. SHIBA volunteers are highly trained to counsel residents of all ages regarding their <br /> choices, options and problems with Medicare, private health insurance,affordable prescription <br /> drugs, and government programs. SHIBA volunteers and staff are also trained to counsel <br /> clients about Medigap coverage,employment-related health benefits, managed care, health <br /> care fraud/abuse, and health care resources for rural, low income and ethnic populations <br /> This agreement will be effective through January 2020 and may be extended,revised,or <br /> amended by mutual agreement with 30 days notice. <br /> To achieve the above, Homage Senior Services SHIBA Program will provide the following <br /> services: <br /> • SHIBA will staff CARL GIBSON SENIOR CENTER with a volunteer(s) who will <br /> provide up-to-date information so that the client can make an informed health care <br /> decision. <br /> • All SHIBA services are free of charge for the clients. <br /> • SHIBA will assist in marketing efforts for the services. <br /> • SHIBA hours will be held the second and fouth Thursday of the month, or at a <br /> time mutually agreed to by the CARL GIBSON SENIOR CENTER and the SHIBA <br /> volunteer. <br /> • Open Enrollment events will be scheduled at mutually agreed upon times. <br /> The CARL GIBSON SENIOR CENTER agrees to: <br /> • The CARL GIBSON SENIOR CENTER will provide adequate facilities to welcome <br /> and seat clients. <br />
