Subject: Proposed Holiday Inn Everett, 3105 Pine Street <br /> September 16, 2005 <br /> Page 2 <br /> 13. Extra wall protection is required on walls behind sinks and food prepazation tables. A 16-inch high <br /> backsplash of plastic laminate, fiberglass-reinforced plastic or equal is acceptable. Floor to ceiling <br /> protection is required on wall behind dishwashers. Wall protection behind mop sinks must cover the <br /> entire splash zone. Plastic coated hardboard is not acceptable. <br /> l4. The floor must be surfaced with a durable, nonabsorbent, easily cleanable material. <br /> 15. The ceiling above the food prepazation azeas must be nonperforated, nonabsorbent, smooth, and <br /> easily cleanable. • <br /> 16. All light fixtures in food prepazation and storage areas must be provided with covers or shatterproof <br /> bulbs. <br /> 17. The font size for the consumer advisory found on the page bottom of submitted menus does not meet <br /> requirements. The minimum font size for a consumer advisory is 11 point. Tiie consumer advisory <br /> on the menus must be changed or one of the options for the consumer advisory noted on the enclosed <br /> consumer advisory information must be used. <br /> 18. Plumbing must meet state and local codes. <br /> 19. The ventilation system shall be installed and operated to meet applicable building, mechanical, and <br /> fire codes. <br /> Additionally please note that prior to operating permit issuance and approval to open the pool and spa, <br /> plans and specifications for the pool and spa must be submitted to the Health District for review. After <br /> tlie Health District plan review process for the pool and spa is completed and construction is finished, <br /> the Health District permit application process must be completed and a preoperational inspection must <br /> be conducted. Please contact Nfike Young at 425339.5250 for pool and spa plan review submittal <br /> information. <br /> A preoperational inspection is required prior to opening for business. At the time of inspection the <br /> construction of the food service establishment must be complete and all equipment must be in place. <br /> Incemplete construction may result in a �149.00 reinspection fee. Contact the Food Program o�ce a <br /> mi�imum of one week in advance to schedule an appointment. This will ensure compliance with the <br /> Rules and Reeulations of the State Board of Health for Food Service Sanitation. <br /> If there are any changes or additions to the approved layout or equipment, the Snohomish Health District <br /> must be notified. - <br /> Please contact me if you have any questions.
