Exhibit "A" Scope of Services, Time of Performance, Budget, Payment, Notices, and Representatives I. Scope of Service A. Activities/Project Description The Subrecipient will be responsible for administering the Child Care Subsidy program in a manner satisfactory to the City and consistent with any standards required as a condition of providing these funds. Such program will include the following activities eligible under the Community Development Block Grant Program: Program Delivery Activity: The Child Care Subsidy will serve Insert number of people to serve 55 persons by providing Enter the program and/or activity descri childcare services for preschool and school age children starting July 1, 2017 through May 31, 2019. Funds will be allocated for low and very low income recipients. The program will meet all state licensing standards and will provide enrichment curriculum, USDA approved snacks, swim lessons where available, and organized on site activities. Participants must register for the program on a monthly basis. Full time status is determined based upon the numbers of days each participant attends in one week. Subsidy for this program will be determined in two ways: 1) participants must apply for financial assistance and must meet the minimum income criteria for their family size and 2); participants must apply for assistance through DSHS and meet their criteria for state funding. General Administration [Description of general administrative services to be performed in support of activities noted above] The Everett Family YMCA's licensed childcare program meets all local and state guidelines. The program is administered by a YMCA professional director who also meets state licensing guidelines. Reimbursement documentation will be provided via the YMCA's general ledger distribution.
