Exhibit "A" <br /> Scope of Services, Time of Performance, Budget, <br /> Payment, Notices, and Representatives <br /> I. Scope of Service <br /> A. Activities/Project Description <br /> The Subrecipient will be responsible for administering the Child Care Subsidy program in a manner <br /> satisfactory to the City and consistent with any standards required as a condition of providing these <br /> funds. Such program will include the following activities eligible under the Community Development <br /> Block Grant Program: <br /> Program Delivery <br /> Activity: <br /> The Child Care Subsidy will serve Insert number of people to serve 55 persons by providing <br /> Enter the program and/or activity descri childcare services for preschool and school age children starting July <br /> 1, 2017 through May 31, 2019. Funds will be allocated for low and very low income recipients. The <br /> program will meet all state licensing standards and will provide enrichment curriculum, USDA <br /> approved snacks, swim lessons where available, and organized on site activities. Participants <br /> must register for the program on a monthly basis. Full time status is determined based upon the <br /> numbers of days each participant attends in one week. Subsidy for this program will be determined <br /> in two ways: 1) participants must apply for financial assistance and must meet the minimum <br /> income criteria for their family size and 2); participants must apply for assistance through DSHS <br /> and meet their criteria for state funding. <br /> General Administration <br /> [Description of general administrative services to be performed in support of activities noted above] <br /> The Everett Family YMCA's licensed childcare program meets all local and state guidelines. The <br /> program is administered by a YMCA professional director who also meets state licensing <br /> guidelines. Reimbursement documentation will be provided via the YMCA's general ledger <br /> distribution. <br />
